Although the thickened liner nlate for the primary shield walls, column _;n_cn_d sister splice assemblies for 83' series cadwelds were stored with rust inhibitin:t paper enclosed inside the sleeves and plastic caps taped over the ends with duct clocure tape, rust is present inside the sleeves.

y Un pronque to clean the _inside of env rasty sleeve by randbinating to r net On i ' t.- M of Specification 7220-C-231Q When cualification tests for cadwelders were made in the vertical position, sister splice assemblies for the
'B' series cadwelds were used.
These vero cleaned by sandblasting, and the inside of the sleeves was as clecn as a new cleeve.
When tested, all four qualification tests ended with fractured rebar, with an average of 99,262 PSI and a low of 96,750 PSI.
This meant that eight out of eight sleeves sandb3asted perfor=ed to the design criteria.

Detail.3 of Dwg. 7220-C-393, Rev. 5 allows a maximum two inch web cope to the support columns for the connection to the horizontal beam.
Coping of the columns in excess of 2 inches was done based on the attached telecon of 12/3/73, which resulted in the writing of the attached FCR-C-26 on 12/5/73.
The telecon did not specifically 7
reference FCR-C-26, which would have provided traceability to the DCN to be issued as required by Section II, No.

" Suitably packaged" can include cardboard containers which are not air tight.
Para. h.3.3 does not include heating require-monts for E6010 and indicates the lack of the technical need for heating.

January 2, 1974 Approximately one quarter of the assemblies with Type B sleeves were tagged.
Janaurv 7, 1974 H. W. Slager discussed tagging of the Type B cadweld assemblies with 3 G. Tucker and J. I. Dotson. At this time it was stated that it was
'ypchtels policy to tag all nonconforming items.

Nonconformance Description and Supporting Details:
Welding of 1,in,er plate leak chase was not being done according to the weld procedure referenced in Specification C-111, Rev. 2.
Leak chase was being welded over a welded section of replacement plate on liner plate S1-1-U2 on 1-9-74.
A worker was observed knocking the coating off a 7018 electrodd.
The welder, (BM-34) then prepared to use the rod as a manually fed filler rod, feeding the rod into the j
cre created by the electrode in the electrode holder. This method is not part of weld procedure P1-A-C-Lh on which the welder was qualified and which is to be used en liner plate.

The Bechtel Senior Field Welding Engineer stopped the man's welding and had the
" slugged" weld removed and repaired.
Further corrective action needs to be taken by Agreed Upon Corrective Action: Bechtel to preclude reoccurrence.
It is recot: mended that corrective action include increased surveillance of liner plate activities by supervisory ~ personnel.

Nonconfomance Description and Supporting Details: Leak chase pressure decay test is not being conducted in accordduce with the controlling documents.
On January 3, 1974, and January 4,1974, a test pressure of 90 PSI was observed on leak chase of. plates on the west jig.
Specification C-111 calls for pressurization to 80 PSI.
Regulatory puide 1.19, which was in effect through a TWX dated December 26, 1973, from P. A.
Martinez to E. E. Felten, calls for pressurization to the containment de' sign pressure (67 PSI).

During a Consumers Power review of the cadweld runspection data, to determine if a more cimplified void measuring technique was feasible, it was found that several void areas j
were in error as a result of simple mathematical calculations (see attached letter).

Th:ise errors occurred in spite of a review of the calculations by Bechtel QC and the, high '
visibility associated with the cadweld reinspection.
It is reasonable to assume that mathematical errors may be discovered in any data,'however, the number of errors found thus far is excessive and a complete check of the cadweld reinspection data may indi-ccte several more errors.
