This refers to the telephone conversations between you and Mr. Heishman of my sta#f on April 23, 1977, and betweca you and Mr. Marguglio and Mr. D. W. Hayes of my staff on April 28 and 29, 1977 relative to the finding by Consumers Power Company that two tendon sheatihs were omitted and two others misplaced in the Midland Cait I containment building.
This finding and the past history of rebar placement errors indicate further evaluation of your QA/QC program may be needed to assura safety related work is accomplished in accordance with your co=mitments and design specifications.
Relative to the above we wish to confirm our undcratanding that 1
Consumer 9 Power Company will:
1.
Notify the NRC Region III office prior to repairs or modifications involving placement of concreto in the arca of the misplaced and omitted tendon sheaths.
2.
Complete its investigation to identify the cause(s) of the tenden sheath deficicacies and impicment required correctiva action.
3.
Pending completion and NRC review of item 2 above, expand "over-view program" for robar placement or otherwise provide for two independent levels of QC inspection to verify that all embedments for Class I structures are properly placed prior to release for concrete placement.

4.
Starting May 9, 1977, and for the next 120 days, notify the NRC Region III office within five days after identification of all embedment placement errors identified af ter final acceptance by the first icvel of QC inspection.
5.
Review and revise as necessary QC inspection procedures to provida for independent verification that placement of embedmonts in in accordance with design courco documents.
