INTERIM REPORT #4 DATE:

Consumers Power Company Midland Plant Units 1 & 2 Bechtel Job 7220 Introduction This report is submitted to advise of the interim status and course of action required pursuant to MCAR #20 and NCR 1185.

The cracked weld repair procedure has been finalized by U.J. Woolley Company upon review by Bechtcl.

To complete the repair procedure, the cracked stiffener repair details being developed by Woolley is

The final report is scheduled for June 15, 1978.

Deficiency Interim Report #1 dated February 17, 1978, reported that inprocess visual inspection revealed that a total of nine vendor welds b'etween the stiffeners and the welding ring were cracked. Magnetic particle _ testing was performed on these welds by Bechtel Construction, and it was determined that the weld for the horizontal stiffener of the personnel lock in Unit

, 2 das not cracked as originally reported (reference Figure 2 of Interim Report #1).

The testing also verified that the remaining eight reportedly crackhd vendor velds had failed.

NCR 1185 reported that one stiffener in the personnel lock of reactor building Unit'l had fractured.

Figure 1 shows the location of the fractured stiffener.

Safety Implications The safety implications have been discussed in Interim Report #2, dated March 31, 1978.

Corrective Action The documentation for the weld and weld test procedures used for the subject fillet welds has been reviewed by project engineering.

It has been determined that groove weld procedures, reviewed and approved by project engineering, were substituted for fillet veld procedures.

June 1, 1978 substitution of procedures is permitted under Section IX, Subsection QW-202.2 of the 1974 ASME Code.

The nondestructive examination technique and the acceptance criteria used for these welds were also found to be sc'ceptable.

The fillet velds between the horizontal stif feners and the weld ring have been inspected by magnetic particle examination, and no cracked weld was discovered.

The cracked welds have been ground out and the gap between the stiffeners and the weld ring has been mapped.

This information was transmitted to 11, 1978.

Consumers Power Company Midland Plant Units 1 & 2 Bechtel Job 7220 Introduction This report is submitted to advise of the interim status and course of action required pursuant to MCAR #20 and NCR 1185.-

The cause of the cracked fillet welds discussed in the following section led to concern as to the adequacy of the remaining welds on'the personnel locks. The personnel locks in Units 1 & 2 reactor buildings were examined by Woolley and Bechtel project engineering on June 12, 1978, for proper size fillet welds.

It was determined that numerous fillet welds were undersized, varying between 1/8 and 1/32 inch short of their specified dimensions. To complete the repair procedure for the personnel locks will require additional investigation of the undersize welds by Woolley and Bechtel project engineering.

The final report is rescheduled for September 15, 1978.

,cause The cause of the cracked fillet welds has been determined by Woolley and Bechtel to have been excessive fit-up gap and insufficient dimensions of the fillet welds which were undersized when' they were not increased to compen' sate for the gap. The cracks developed when these welds were stressed during welding between the weld collar and the reactor building penetration sleeve.

Deficiency Interim Report #4, dated June 1, 1978, reported on the deficiency as described on NCR 1185. Further investigation of the welds on the personnel locks determined that other fillet welds are undersized.

The extent of the undersized welds will be determined when the personnel locks are reinspected by Bechtel Quality Control.

Attcchment to BLC 5122 Bechtel Associates Professional Corporation MCAR #20 INTERIM REPORT 5 Page 2 June 15, 1978 Safety Implications y,J. Woolley has been requested to evaluate the integrity of the personnel locks taking into consideration the deficient welding.

This evaluation will be performed after Bechtel Quality Control has determined the extent of the deficient velds.

Corrective Action Bechtel Quality Control has been requested to reinspect the personnel locks and their documentation to determine the extent of the deficiencies, including the undersize welds. Their response has been requested by June 28, 1978.

Woolley has developed a procedure for repairing the cracked stif fener described in Interim Report #4. The repair procedure has been incorporated into the personnel' lock weld repair procedure.

The procedure is being reviewed by project engineering.
