DATE OF EVENT _:
April 29, 1977 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: While performing the Nuclear Instrumentation Pre-Op, a test signal was inserted to the power range channel which caused high voltage cut-off to both source range instruments.
Conditions Prior to Occurrence: The plant was in Mode 6 with the Reactor Vessel Head off.
No core alterations were in progress.

Descriptio'n of Occurrence:
At 1010 hours0.0117 days <br />0.281 hours <br />0.00167 weeks <br />3.84305e-4 months <br />, while performing the Nuclear Instru-mentation Pre-Operational Test, a test signal was inserted into the power range channel which corresponded to a power level of greater than 10%.
This caused the automatic high voltage cutoff to occur in both source range instruments
, which_placed the station in an Action Statement for Technical Specification 3.9.2.
At 1012 hours0.0117 days <br />0.281 hours <br />0.00167 weeks <br />3.85066e-4 months <br />, the source range instrumentation was resto. red.

have been caused by a procedure inadequacy in the Nuclear Instrumentation Pre-

Operational Test, TP 301.01.
Analysis of Occurrence: Since no core alterations were being performed and since a boron concentration of 2* 1800 ppm boron was maintained, the health and safety of the public or station personnel was not threatened.
Corrective Action: A modification was made to the procedure to prevent automatic cutoff of the high voltage to the source range instruments during this test.
Failure Data:
No previous similar events have occurred.
