Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE:

Post Accident Radiation Monitor RE 5029 Failure Conditions Prior to Occurrence: The unit was in Mode 1, with Power (MWT) = 2080

and Load (MWE) = 727.

Description of Occurrence: On March 9,1978, Post Accident Radiation Monitor RE 5029 was found inoperable due to low sample flow as indicated on pump panel RCM 5029. This placed the unit in the Action Statement of Technical Specifica-tion 3.3.3.6, which requires the functional unit to be repaired in 30 days.

The Technical Specification requires both Post Accident Radiation Monitors while in Modes 1, 2 or 3.

During the preparation of this report,.it was recognized that nu=erous previous failures had occurred but had not been reported.

The apparent cause of the incident on March 9,1978, is due to pu=p motor (Reliance Part #14G4025-A5) bearing seizure.

This seizure is believed to be caused by the high anhient tenperature conditions inside the radiation monitor cabinet which contains the entire monitor.

There was no danger to the health and safety of the public or to unit personnel.

These instruments do not control any equipment but are used for monitoring purposes only.

No incident requiring use of this instrumentation occurred during the time the monitors were inoperable.

Corrective Action

It was decided in conjunction with Victoreen to replace the motor bearings with a sealed type bearing, FAFNIR Part No. 203PP and 205PP.

The motor bearings were replaced per Maintenance Work Order 78-579 in RE 5029. Post Accident Radiation Monitor RE 5030 is in a penetration room with lower ambient temperature; it has not been necessary to replace its motor bearings. Facility Change Request 78-159 was prepared for replacement of these bearings in all process monitors of this type.

The reporting require =ents of this Technical Specification will be emphasized with all affected station personnel.

Previous motor failures have occurred on Septe=ber 8,1977 and

( V by various equipment')1fovenber 8,1977, but only on RE 5029.

The re=aining occurrences were caused failures.
