DAVIS-BESSE UNIT ONE NUCLEAR POWER STATION SUPPLEMENTAL INFORMATION FOR LER NP-32-77-5 DATE OF EVENT: June 12, 1977 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: Loss of decay heat flow caused by personnel error during the performance of the SFAS Monthly Test.
Conditions Prior to occurrence: The plant was in Mode 5.

-Description of Occurrence: At 2222 hours0.0257 days <br />0.617 hours <br />0.00367 weeks <br />8.45471e-4 months <br /> on June 12, 1977, while performing Steps 4
j 6.3.1 through 6.3.4 of the "SFAS Monthly Test", ST 5031.06, Module BA 413 reset by l
the RCS pressure test switch and closed DHil to isolate the suction of the operating decay heat pump in violation of Technical Specification 3.4.1.
The pump was tripped, i

flow was re-established.
Designation of Apparent Cause of Occurrence: The cause of this occurrence has been determined to be personnel error.
A modification to the "SFAS Monthly Test" was i
noe followed.

I Analysis of Occurrence: Since flow was not needed for decay heat removal and the Decay Heat Pump was restarted, no threat resulted to the health and safety of the public or to station personnel.
Corrective Action: The modification was posted at the SFAS abinet and operators were instructed to be cognizant of all =odifications to a procedure before proceed-ing with the performance of that procedure.
4 Failure Data: DH11 was closed on two previous occurrences (NP-32-77-2 and NP-32-77-3) but the cause for closure was not the same since one was caused by a wiring insula-i tion error and.the other by a procedure error.
