SUPPLEMENTAL INFORMATION FOR LER NP-33-77-84 g

DATE OF EVENT: October 25, 1977 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: The Surveillance Test ST 5080.01 was run a day late.

Conditions Prior to Occurrence: The plant was in Mode 3, with Power (MWT) = 0 and Load (MWE) = 0..

Descrintion of Occurrene,-

During the review of the Surveillance Test Schedule by the Surveillance Test F.agineer, it was discovered that ST 5080.01, Independent Offsite AC Source Line-u; and Availability, had not been perforned by its late date, to meet the frequency requirements of the Technical Specification 3.S.1.1.

Designation of Anoarent Cause of Occurrence: The cause of this occurrence is personnel error. While <.2cking off completed Surveillance Test on the operator's working copy of the Survt illance Test Schadule. the operator mistakenly " checked off" ST 5080.01 as being. crformed instead of ST 5081.01. Also, when the

Surveillance Test Enginece reviewed the Surveillance Test Schedule on October 24, s_,/

1977 this test was not recognized as being required on that day.

Analysis of Occurrence: There was no danger to the health and safety of the public or Station personnel. The Diesel Generators were operable if there had been a loss of the off-s te power supply and the need for the safety related equipment for the mitigation and control o~f accident conditions.

Corrective Action

The ST 5080.01 was immediately performed, and a computer program change to the Surveillance Test Program was already being made. The change will enable the Surveillance Test Engineer to know exactly which tests are required on a given days The Shift Foreman will then be informed of these tests.

In this way, any discrepancy between the working schedule and the test required that day will then be known before the tests are late.

Failure Data: There have been no previous similar occurrences.
