Dear Mr. Keppler:

Toledo Edison acknowledges receipt of your February 20, 1974 letter and enclosure referencing apparent violations of AEC requirements and non-conformances with our quality assurance program. The two items resulted from site inspections conducted by your office on January 8-10, 1974.

Following a thorough review of the two items of concern, Toledo Edison

(/,_s) offers the following information regarding the apparent violations, including corrective actions and steps taken to avoid further repetition.

s, Item A.1 - Written Weld Inspection Procedures Or Instructions - Criterion V.

A stop work order was issued on January 8, 1974 against Fischbach and Moore /

Colgan Electric (F&M/C) regarding F&M/C field welding.

The sep work order was based on findings resulting from an audit performed by Toledo Edison's Quality Assurance implementation agent, Bechtel.

F&M/C honored the stop work order immediately and proceeded to in6;ctri-nate personnel and implement written procedures contained in the Toledo Edison - accepted F&M/C Quality Control Manual, requiring the use of a documented inspection plan.

On January 31, 1974, a follow-up audit was performed which indicated that F&M/C vere then prepared to conduct their activities in compliance with their Quality Control Manual regarding inspection activities in accordance with detailed procedures.

Following several joint meetings, wherein mutual understanding and re-solution was accomplished, the Toledo Edison and Bechtel Quality Assurance Engineers agreed on February 6, 1974 to release F&M/C from the stop work order, through the Project Construction Manager.

, Page 2 March 22, 1974 AEC-DRO Region III inspectors, Messrs. Dickerson and Hayes, reviewed the actions described herein, on their site visitation of February 13-15, 1974.

Item A.2 - Documentation of ControlJed Welding - Criterion V.

The response listed for item A.1 also applies hereunder.

Further corrective action has been accomplished at both F&M/C and Bechtel, regarding documented results of inspections and welding operations.

F&M/C implemented in late February, a written detailed instruction, issued to their site organization covering welding operations and attendant inspection requirements.

Bechtel welding engineer inspection activities have been reinforced with indoctrination and positive written direction regarding monitoring activ-ities of F&M/C welding.
