Toledo Edison acknowledges receipt of your March 20, 1974 letter and enclosure referencing an apparent violation of AEC Requirements and non-conformances with our quality assurance program.
The item resulted from a site inspection condncted by your office on February 13-15, 1974.

'V Following a thorough examination of the item of concern, Toledo Edison offers the following information regarding the apparent violation, includ-ing corrective actions and steps taken to avoid further repetition.
Item A - Documantary Evidence - ASME Manufacturer's Report - Criterion VII On February 25, 1974 the ASME Manufacturer's Data Report was filed in the site Babcock & Wilcox data package for the decay heat exchanger, DH-HX-1B.
The Quality Assurance Release was amended on February 25, 1974 to indicate the actual existence of the ASME Manufacturer's Data Report.
The exchanger was initially teleased and installed on the basis of a Quality Assurance Release provided by Nuclear Power Generation Division of B&W-Lynchburg, Virginia, indicating that requirements of the B&W purchase order had been met.
An extensive review was performed by Babcock & Wilcox Nuclear Power j
Generation Division Quality Assurance Representatives of all NSSS supplied data packages at site. This review was completed on March 29, 1974.
