APPENDIX A NOTICE OF VIOLATION l

Omaha Public Power District Docket:

50-285 Fort Calhoun Station Operating License:

DPR-40 During an NRC inspection conducted October 10-13, 1989, a violation of NRC requirements was identified. The violation involved a failure to follow procedures designed to identify personnel contamination.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions,"-

10 CFR Part 2, Appendix C (1989) (Enforcement Policy), the violation is listed below:

Section 5.8.1 of the Technical Specifications states, in part, that l

written procedures shall be established, implemented, and maintained that meet or exceed the requirements of Appendix A to Regulatory Guide 1.33.

Paragraph 7 of Appendix A to Regulatory Guide 1.33 states, in part, that the control of radioactivity (for limiting materials released to the environment and limiting personnel exposure) shall be covered by written procedures.

Standing Order No. G-76 requires that all personnel exiting the restricted area via the security building pass successfully through the portal monitors.

Contrary to the above, on September 20, 1989, a group of approximately 40 visitors who had been within the restricted area were allowed to leave i

the site without first being checked for radioactive contamination.

This is a Severity Level IV violation.

(Supplement IV) (285/8941-01) t Pursuant to the provisions of 10 CFR 2.201, Omaha Public Power District is hereby required to submit to this office,.within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply,

' including for each violation:

(1) the reason for the violation if admitted, i

(2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, i

consideration will be given to extending the response time.
