Violation 369.370/89-24-01 D
. Licensee Technical Specification Table 6.2-1 requires a minimum u
L shift crew, when either unit is in Mode 1 through Mode 4, of one l'
shift Snpervisor, one Senior Reactor Operator, three Reactor l
Operators, three Auxiliary Operators, and one Shift Technical l
' Advisor.
NUREG 0654 states that this is the minimum shift necessar7 to assure the safe shutdown of a two-unit plant, when l
one of the plants is in an emergency. condition.
Selected Licensee Commitment (SLIC) 16.13-1, Fire Brigade, E
requires that.a Fire' Brigade of at least five members shall be maintained.onsite at all times.
The Fire Brigade members cannot include' members:of the minimum shift crew necessary for the safe shutdown of the plant.
Licensee Procedure, OMP 2-2, Shift Turnover, requires that each' shift verify that minimum shift staffing requirements and Fire Brigade staffing requirements are met.
. Contrary to the above:-

During the. periods of June 1989 to August 1989, the licensee routinely failed to follow Procedure OMP 2-2 by not properly documanting the minimum shift crew's presence.
No objective evidence exists that the Technical Specification requirements and the SLIC requirements were being met.

Licensee Procedure OMP 2-2, Attachment la, was inadequate, in that it allowed members of the minimum shift staff to also be assigned to the Fire Brigade.
In the event'of an emergency situation involving a fire, the ability of both the Fire Brigade to respond to the fire and'the minimum shift staff to bring both units to safe shutdown may have been reduced.
This is a Severity Level IV (Supplement 1) violation.
