l During an NRC inspection conducted September 1 through October 10, 1989, violations of NRC requirements were identified. The violations involved the failure to issue monitoring devices to all onsite personnel, announcement of the arrival of an inspector on site, and issuance of a security badge to an unqualified individual.

't and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1989)

(Enforcement Policy), the violations are listed below:

A.

Failure to Issue Monitoring Devices to All Onsite Personnel Section 50.54(q) of 10 CFR Part 50 states, in part, that a licensse authorized to operate a nuclear power reactor shall follow emergency l

Emergency Response Plan states, in part, that all individuals inside the restricted area are required to wear personnel monitoring devices capable of measuring the dose received from external sources of ionizing radiation.

L Contrary to the above, on September 12, 1989, the inspector noted that six of ten individuals surveyed were not wearing personnel monitoring devices capable of measuring the dose received from external sources of radiation i

because the personnel had not been issued devices by the licensee.

Announcement of the Arrival of an Inspector

[

Section 50.70(b)(4) of 10 CFR Part 50 states, in part, that the licensee shall ensure that the arrival of an NRC inspector is not announced or otherwise communicated by its employees or contractors unless-specifically requested by the NRC inspector.

Section 7.1 of Procedure SECOP-6, " Personnel Portal Control," states, in part, that upon arrival by any representative of the NRC, there shall be no notification given by security.

Contrary to the above, on September 18, 1989, the arrival of an inspector on site was announced by security personnel over intercom radios without i

being specifically requested by the inspector.

Issuance of a Security Badge to an Unavalified Individual l

License Condition 3.c of the Fort Calhoun Station Operating License, dated March 25,1981, requires that the licensee maintain in effect and fully i

' implement all provisions of the Commission-approved Physical Security Plan i

including amendments and changes made pursuant to the authority of 10 CFR Part 50.54(p).

Section 7.2.3.1 of the licensee's Site Security Plan states, in part, that I

NRC employees will be provided unescorted access to the Fort Calhoun Station upon verification of their identity and successful completion of the general employee training program, j

Contrary to the above, on September 28, 1989, an NRC employee, whose i

general employee training qualifications had expired in April 1989, was granted unescorted access to the facility.

This is a Severity Level IV violation.

Pursuant to the provisions of 10 CFR Part 2.201, the Oc.aha Public Power District is hereby required to submit to this office, within 30 days.of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation:

(1) the reason for the violation if admitted, (2) the corrective steps that have been taken and the results dChieved,'(3) the Corrective steps that will be taken to avoid further i

violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
