RSLO Counterpart Meeting ARegionalStateLiaisonOfficers(RSLO)counterpartmeetingwasheldwiththe State Local and Indian Relations staff at Headquarters on October 16-17,1989.
The meeting provided an opportunity to review policy guidance and discuss issues of interest with other NRC staff. Harold Denton, Director of Governmental and Public Affairs, discussed the recent State Liaison Officers Michael meeting held in Region II and items of interest from that meeting.
Bell, Chief of the LLW Regulatory Branch, gave a status report onJohn Greeves Deputy decommissioning-related activities within the Commission.
Director of LLWM, Paul Lohaus, Chief of the LLW Operations Branch, and Rob MacDougall, LLWM, reviewed the status of a variety of LLW-related issues including Governors' certifications, storage options and guidance development, mixed waste, greater than Class C, and the recent National Governors' Association staff meeting on LLW. Bill Lahs RES, made a presentation on the status of activities on the regulatory exemption policy. During a discussion on implementation of the Commission policy on cooperation with States, John

San Francisco Earthquake 17, 1989, theBoiseInteragencyFireCache(BIFC)wascalledat On October 9)m EST by the Telecommunications Branch and put on stand-by because of tie earthquake in the San Francisco area. BIFC is under contract to provide NRC with emergency telecommunications. Region V only had its one dedicated voice line working back to the NRC Operations Center. The Regions commercial ) hone service was restored by 11pm EST and BIFC was released from stand->y status. All of the Emergency Notification System (EN lines to power plants remained in service. The AUTO-DIAL Network System NS) lines to the Region also remained in service.

v Office of Nuclear Reactor Regulations Items of Interest Week Ending October 20, 1989 Carolina Power and Light Meeting: AFW System SPLB, DPL II'-1 and Region II met with Carolina Power and Light on October 10, 1989 to discuss the licensees corrective actions for the auxiliary feedwater
( AFW) system NPSH problem at H.B. Robinson. The licensee described the proposed modifications including increasing the AFW pumps suction piping size and other AFW system modifications to bring the AFW system back into conformance to with its design basis requirements. The staff requested some additional verification be made by the licensee of assumed AFW flow requirements for various events, but was in agreement that the proposed actions are adequate to ensure proper pump performance and therefore, restart could commence upon their completion. AFW system testing will be performed prior to and during restart power ascension. Restart was scheduled for later in October 1989.
LPCI Single Failure. BWR 4 Plants On October 6, 1989, Fermi-2, Monticello, Dresden and Quad Cities reported a potential single failure problem that could result in a complete loss of the LowPressureCoolantInjection(LPCI)functionduetoalowvoltagecondition on the LPCI bus. The low voltage condition could result from a postulated failure of an Emergency Diesel Generator Voltage regulator. The same EDG voltage voltage regulator failure would also result in degradation or failure of one division of core spray. These postulated ECCS failures, in the event of a large break LOCA, would result in an inadequate core cooling event in BWR-4 plants.
INPO has sent notification of this potential failure mode to the affected BWR-4 licensees. SELB is working with Reactor Projects to establish an NRR course of action.
Maine Yankee - Noise at Hot Leg loop Isolation Valve l
1 During the week of September 18, 1989, the licensee detected an increased level of noise from the No. I steam generator acoustic monitoring system.
After installing additional instrumentation, the licensee determined that the noise was coming from the hot leg isolation yhlve.
)
During a reactor shutdown on October 10, 1989, the operators, following l
procedure, took the #1 hot leg loop isolation valve off its back seat to l
prevent thermal bir, ding while cooling down the plant. Consequently, the noise that had been coming from the valve ceased. An inspection conducted after the l
L plant shutdown indicated the presence of boron precipitate on the valve and on r
the floor underneath. The licensee suspected that a valve packing leak was l
the cause of the excessive noise. The packing was adjusted and, following I
startup on October 16, the excessive valve noise had not recurred.

L to discuss the GA-4 and GA-9 spent fuel casks. These casks are being designed for the Department of Energy for transport of spent fuel by truck under the Nuclear Waste Policy Act. The structural design of the package, and the basis i
for evaluating its thermal, criticality, and shielding safety were discussed at l
the meeting. An application is expected to be submitted to the NRC in 1992.
LOW-LEVEL WASTE MAhAGEMENT Siting Requirements Briefing for New Jersey LLW Disposal Facility Siting Board On October 12, 1989, a staff member of the Division of Low-Level Waste Management and Decommissioning attended a meeting of the New Jersey Low-Level Radioactive Waste Disposal Facility Siting Board to discuss the requirements of 10 CFR Part 61 and answer questions about the siting process. The Siting Board is charged with developing a low-level waste disposal facility for the State to I
carry out the requirements of the Low-Level Radioactive Waste Policy Amendments l
Act of 1985 and commitments made to the Northeast Compact. The Board is moving forward rapidly to site a disposal facility. They were particularly interested in the National Environmental Policy Act (NEPA) process, the availability of NRC resources to continue effective pre-licensing interactions and NRC staff considerations concerning the siting process.

Integrity of Reactor Components Part 21 Notification for IAV Relays In an October 13, 1989 letter to the Director, NRR, the General Electric Company indicated that the published seismic ratings of IAV relays in the operating mode with normally open contacts are now in question due to a review of a 1977 GE test report. The 1977 GE test report was being reviewed because testr recently performed by Wyle Laboratories for Brookhaven National Laboratory (BNL) under NRC/RES sponsorship could only support a seismic rating approximately 20% of the published levels. When presented with the BNL data, GE reviewed their seismic test reports and discovered an error. GE has notified +, hose nuclear units that purchased these IAV relayt as class IE that a problem exists and has committed to additional testing.
IAV relays are being reviewed as part of USI A-46 and are expected to be included in the seismic portion of IPEEE.
Resolving Safety Issues and Developing Regulations Generic Issue B-56 Diesel Generator Reliability The staff met with the ACRS Subcommittee on AC/DC Power Systems Reliability on October 2,1989 and with the full Comittee on October 6,1989 to discuss the resolution of GSI B-56, " Diesel Generator Reliability" through the issuance of Regulatory Guide 1.9, Revision 3.

staff also presented their views with respect to this regulatory guide at these meetings. Because insufficient time was allotted for discussion at the full committee raeeting, the staff will be asked to return to the full L

The staff also met with the CRGR on October 11, 1989 to discuss the resolution I
of B-56. As a result of this meeting, the staff will be requested to return l-to the CRGR to further discuss RG 1.9, Revision 3 (per suggestions made by L
CRGR members at the October 11, 1989 meeting) and specific implementation actions associated with the regulatory guide.
Future Meeting: Public Workshop on Technical and Policy Considerations for Nuclear Power Plant License Renewal In a Federal Register Notice dated October 13, 1989 (FRN Vol. 54 No. 197 pp. 41980-41986 (inclusive)), the NRC announced a public workshop on Technical and Policy Considerations for Nuclear Power Plant License Renewal. Future Meeting: Public Workshop on Conversion to the Metric' System E-In a Federal Register Notice dated October 16,1989(FRNVol.54,No.198 pp. 42426-42427) the NRC announced'a public worksho) on conversion to tne metric' system. NRC staff will collect information ttat will assist the NRC in developing'its policy and plans for complying with the Omnibus Trade and' Competitiveness Act of 1988. The workshop will be held on November 14 and 15, 1989 at the Holiday Inn-Inner Harbor in Baltimore.. Maryland.

t Items of Interest Week Ending October 20, 1989 Division of Operational Assessment (DOA)
At approximately 1703 pacific day light time (PDT), an earthquake with an estimated level of 7.0 on the Richter Scale occurred with an epicenter approximately 10 miles northeast of Santa Cruz, California, near the San Andreas Fault. Additional Operations Officers were brought to the Operations Center to handle the communications with NRC management, Region V, licensees, other federal egencies and international agencies.
IR8 determined the status of all power, test and research reactors and communicated the results to appropriate contacts inside and outside the NRC. An o>en line was established and maintained throughout the night with Region V whic1 manned its Incident Response Center. During this time the normal commerical telephone system could not be used because it was overloaded. However the Federal Telecommunication System (FTS) and the Emergency Notification System (ENS) tie-line and the teleconferencing system remained available for communications with the response center.
On October 18, the AEOD Director, Arkansas Nuclear One (ANO) Diagnostic Evaluation Team (DET) Manager and Deputy DET Manager, together with representa-tives of NRR and Region IV personnel met with the Arkansas Power & Light Company (AP&L) Chief Executive Officer, Nuclear Vice President and other AP&L executives and Entergy Corporation executives to discuss the results of the ANO Diagnostic Evaluation.
On October 16-17, members of the Palo Verde Diagnostic Evaluation Team met at the AEOD's offices in Bethesda, Maryland. At this meeting the team mem'ers o
met each other and the specific objectives, methodology, program schedule and administrative details for the Palo Verde diagnostic evaluation were discussed.
In addition, detailed presentations on key licensee staff, site and corporate programs and plant performance were provided by the Director, Division of Reactor Projects, Region V, the NRR Project Managers and key members of AEOD's Trends and Patterns Analysis Branch.

Officer, participated in a workshop for New Jersey LLW generators on October 20, 1989 in Princeton, New Jersey. The purpose of the meetinI, sponsored by the New Jersey low Level Radioactive Waste Disposal Faci ity Siting Board, was for the State to discuss onsite storage options and other alternatives with New Jersey's LLW generators. New Jersey officials held this workshop to gain additional information for the preparation of the Governor's certification.

i Two speed bumps have been installed along the driveway entrance to the OWFN garage and two pedestrian crosswalks have been striped at the rear of the building. The inclusion of there enhancements were added to protect the NRC employees when entering and exiting from the garage and loading dock areas.

OMB Cle:rance Actions General Electric Nuclear Energy submitted a letter to the Office of 5
Management and Budget (OMB) stating that the NRC'c request for renewal of the clearance for NRC Form 741, " Nuclear Material Transaction Report" should be denied. GeneralElectric(GE)believesthattherevised NUREG/BR-0006, " Instructions for completing Nuclear Material Transaction i
Reports and Concise Notes" contains numerous errors, requires significant changes to computer systems, and has no implementation date, and that industry war not allowed to comment on the document prior to publication.
The GE letter is being reviewed by NMSS and OGC so that they can prepare a response.

TVA Request to Submit Microfiche FSARs in Lieu of Hard Copy IRMB received a request from Suzanne Black, Assistant Director for TVA Projects, NRR, for the requirements to submit FSARs in microfiche. TVA envisions submitting a complete revised FSAR for each amendment they issue and would like to make the submittals only in microfiche.
IRMB will provide specifications and copy requirements guidance consistent with applicable regulations and Generic Letter 82-14 after copy recipients are polled for need for hardcopy as opposed to microfiche and the availability of Microfiche Reader Printers.

PILGRIM EXERCISE j
A full-participation exercise was held at Pilgrim on 10/12-13,1989.
The second day activities were limited to off-site demonstration of recovery and reentry. The NRC observed on-site portions of the exercise and proviJed an assessment of licensee performance to the licensee on 10/13/89. The licensee demonstrated the ability to implement the on-site portions of its plans in a manner which would have provided prompt, appropriate protective action recommendations to off-site authorities.
No exercise weaknesses were identified by the team of NRC observers.
FEMA Region I held a public meeting at Memorial Hall in Plymouth, Massachusetts on Monday, 10/16 at 7:30 p.m. to present the observations made by FEMA during the Pilgrim full-participation exercise conducted on 10/12/89. FEMA spokesman Jack Dolan indicated that performance had improved in the areas which were identified as weaknesses during previous exercises, and that the final exercise report should be issued in about three months. Craig Conklin, the NRC inspection team leader, represented the NRC at the public meeting and indicated that the licensee's on-site performance was acceptable to protect the health and safety of the public.
Each state agency and local government which participated was provided an opportunity to comment on performance during the exercise.

The licensee conducted the second (and final) shase of the Shutdown from Outside the Control Room demonstration on Octo)er 13, 1989. The test successfully demonstrated the licensee's ability to place the plant in cold shutdown using the alternate shutdown panels outside the control The plant will remain in cold shutdown for about 18 days for a room.

MILLSTONE 2 ALLEGATION FOLLOWUP INSPECTION RESULTS On October 11, 1989, the NRC issued inspection report 50-336/89-13 con-cerning a series of allegations made over the past two years by three OCTOBER 20, 1989 ENCLOSURE N

c Millstone Unit 2 employees. These allegations concerned a number of safety and regulatory issues as perceived by the allegers which had not i
been properly resolved by the Northeast Utilities management. The allegers 1
were interviewed by the NRC over a three day period in June 1989 to further define and clarify the allegations and these interviews were transcribed. A seven person team performed an inspection at Millstone Unit 2 July-10-21, 1989 to resolve technical issues raised by the allegers.
Allegations of harassment and intimidation (H&I) had been previously referred to the Department of Labor for action and they were also referred to the NRC's Office of Investigation for possible NRC followup. HAI allegations were not considered by the technical inspection team.
There were 130 separate allegations of which 18 were beyond the scope of the inspection. Since some allegations were duplicate, 77 separate issues were inspected. The inspection substantiated 34, and partially substan-tiated 24 allegations.
Fourteen allegations were unsubstantiated and three were indeterminate.
Some of the substantiated. issues resulted in five violations which deal l-with 1) multiple examples of failure to follow procedures 2) lack of seismic documentation for an electrical conduit run 3) technicians using outdated drawings 4) failure to functionally test a radiation monitor alarm and 5) improper control of overtime. None of the violations, either singularly or collectively, represented a major safety issue. The 11cen-see was criticized for failure to take a pro-active stand concerning the allegations, and the allegers were criticized for failing to make use of l
the licensee's corrective action systems. The team concluded that future allegations of this nature should be referred to the licensee for disposi-
. tion, and the NRC should do periodic followup of the adequacy of licensee actions.
Since the allegers provided the news media with their allegations earlier I
in 1989, there was considerable interest by the news media regarding the l
NRC inspection results. Articles appeared in the Hartford Courant, the New Haven Register, and the New London Day with quotes from the alleger, the licensee, and the NRC. The licensee expressed satisfaction with the report stating they would take corrective actions. The allegers expressed i
some dissatisfaction with the outcome in that they thought the NRC should have taken more forceful action'with the licensee.

On October 17, representatives of Virginia Electric and Power Company were in the Region II Office to attend an Enforcement Conference regarding maintenance-related corrective action problems and radiation area access control problems at their Surry Nuclear Power Station.

CP&L Management Meeting On October 17, representatives of the Carolina Power and Light Company were in Region II to attend an Management Meeting regarding their nuclear prioritization program and certification of their Brunswick simulator.

Florida Power Corporation On October 18, the Regional Administrator and selected members of the Region II and NRR staffs were at the Crystal River Nuclear Power Station to review plant operations, configuration management control, and various electrical issues.

Duke Power Compang On October 20, the Regional Administrator met with the Vice President, Nuclear Production, Duke Power Company, to discuss issues pertaining to the operation of Duke Power Company nuclear facilities. Representatives of the Duke Power Company were also in the Region II Office to attend an Enforcement Conference regarding recent problems associated with the control room ventilation system and the annulus ventilation systems at their McGuire Nuclear Power Station.

Enforcement Conference - Hospital de Damas On October 20, the Director, Division of Radiation Safety and Safeguards, and selected members of the Region II staff were at Hospital de Damas in Ponce, Puerto Rico to attend an Enforcement Conference regarding radio-logical safety problems identified during an inspection conducted on August 30 and 31, 1989.

o the tour, a meeting was conducted with the licensee, in whici the licensee described performance improvement programs they had initiated and were implementing. These programs were addressed towards improving. operator morale, improving staff accountability and overall improvements in the area of plant maintenance.

Uniroyal Goodrich Tire Co.
Mr. Charles E. Norelius, Director, Division of Radiation Safety and Safeguards, and members of the Region III staff met with licensee management representatives on October 17, 1989, to discuss NRC findings t
during a special inspection, enforcement options available, and the licensee's corrective actions. The special inspection resulted from the each (one was subsequently recovered)g about 12 mil 11 curies of Sr-90 reported loss of two gauges containin An in depth review of the program identified several violations. Enforcement action is pending.

Quad Cities On October 18, 1989, Mr. T. O. Martin, Deputy Director, Division of Reactor Safety, and members of his staff met with representatives of Commonwealth Edison Company for the exit meeting on the Maintenance Team Inspection that had just been completed at the Quad Cities facilities.

Midwest Nuclear Training Association On October 17, 1989, Mr. T. M. Burdick, Chief, Operator Licensing Section 1 Region III, gave a presentation at the annual Midwest Nuclear Training Association instructor workshop in Columbus, Ohio. Subjects covered included recent NRC initiatives, requalification program evaluations and 10 CFR 55 concerns.

Zion Nuclear Power Station On October 6,1989, the licensee began an investigation of potential tampering of a printed test circuitboard. The licensee expanded their investigation on October 12, 1989, to include potential tampering with a vent screw on a steam generator wide range level transmitter, which was found loose. The vent screw was located near the same area where the circuitboard was found.
It was later determined by laboratory analysis that the circuitboard had been cut by sidecutters and that maintenance work had been recently performed on the vent screw. The licensee has OCTOBER 20, 1989 ENCLOSURE N

Monticello Nuclear Generating Station The licensee identified a single failure concern regarding a degraded j
condition of an Emergency Diesel Generator during a LOCA with a loss of off site power. This undervoltage or underfrequency condition may 1
prevent certain safety related equipment-from functioning and may also prevent the Low Pressure Coolant Injection (LPCI) system swing bus from transferring to an alternate power supply. The licensee plans to resolve this situation, which appears to be generic to other BWRs with the GE swing bus design. NRR has been informed of this issue.

In response to NRC concerns regarding the failure of a secondary containment integrity test at the Duane Arnold plant, the Monticello plant conducted similar tests to verify the integrity of their secondary containment. With the plenum exhaust fans secured, the vacuum in the reactor building failed to reach the Tech Spec required 0.25 inches water vacuum. The licensee is investigating this event and has initiated actions to resolve the testing deficiencies and repair leaks in the secondary containment system.

Dresden and Quad Cities Nuclear Power Stations

In response to problems at Monticello and Duane Arnold regarding secondary containment and its testing, the Dresden and Quad Cities resident inspectors have reviewed the ventilation system te Standby Gas Treatment (SBGT) system interaction.
So far, this review has found that when SBGT is activated, normal ventilation is automatically isolated, which prevents undesirable interaction between systems.

Commonwealth Edison reported a potential failure of an emergency diesel generator voltage regulator which would prevent LPCI injection during a LOCA combined with loss of offsite power. This problem was originally identified by Monticello. Regional staff and resident inspectors reviewed the implementation of interim corrective actions and an analysis j
of the potential for this failure to occur. The analysis presented an extremely low probability for this event.

San Francisco Area Earthquake Update There continue to be numerous earthquake af tershocks, reported to be about 1400. They have varied in size with a few measured at 5 on the.
Richter Scale. There has been no reported damage to any of the. power, t
. test and research reactors as a result of the aftershocks. Region V contacted the larper NRC materials licensees in the area and one reported a very minor spil, (microcuries of P-32) in a hood. All others reported i
no radiological problems associated with the licensed material as the result of the large earthquake. The State of' California reported to Region V that a check with 12 of the 13 state licensees in the area disclosed no radiological problems occurred or incidents reported resulting from the earthquake. The thirteenth licensee has a type "B" license which has'a limited possession limit.

Sacramento Municipal Utility District (Rancho Seco)
Rancho Seco entered the refueling mode of operation on October 14, 1989, l
by commencing detentioning oof the reactor vessel' head. Detentioning was completed on October 18, 1989. The licensee's present schedule is to remove the fuel from the reactor vessel during the period of November 15-22, 1989.
