Docket No. 99900505/80-01 NOTICE OF DEVIATION Based on the results of an NRC inspection conducted on April 21-25, 1980, it appeared that certain of your activities were not conducted in full compliance with NRC requirements as indicated below:
A.
Ebasco Topical Report ETR-1001, Revision 8A, "Ebasco Nuclear Quality Assurance Program Manual," Section QA-II-5, Revision 1 (Supplier Surveillance), paragraph 4.1, states in part, "The Supplier shall be required to submit (via Purchase Order requirements) a detailed fabrication sequence showing required tests and inspections."
Contrary to the above, on the following contracts examined by the inspector the purchase orders did not call out this requirement:

The "Ebasco Nuclear Quality Assurance Manual," ETR 1001 as supplemented by " Manual of Procedures," Volume 2, WPPSS Nuclear Projects Nos. 3 and 5 describes the Quality Assurance Program in effect on WPPSS Nuclear Projects Nos. 3 and 5.
The " Manual of Procedures" either contains or references procedures and/or instructions for implementing contract obligations and defines any deviations and/or modifications from the "Ebasco Nuclear Quality Assurance Program Manual" in order to satisfy unique project conditions.
"Ebasco Nuclear Quality Assurance Manual," ETR-1001, Section QA-II-5, paragraph 4.2.1 states in part, "The Project Quality Assurance 1
Engineer is responsible for obtaining review and comments on each j
required procedure from the individual or group in Ebasco having expertise in the subject area."
Contrary to the above, the Project Quality Assurance Engineer is not obtaining review and comments on each required procedure, nor has this requirement (QA-II-5, paragraph 4.2.1) been modified I
in the " Manual of Procedures," Volume 2 for WPPSS Nuclear Projects Nos. 3 and 5, eermitting this responsibility to be assumed by others.
