-APPENDIX A NOTICE'0F' VIOLATION Based on the results of the NRC inspection conducted during the period May 5-9, 1975, it appears that certain of your activities were not conducted in full compliance with your Reactor License or NRC Regulations as indicated below:
DEFICIENCIES:
4 Contrary to ArkanIsas Nuclear One - Unit 1. requalification program (, approved-November 25, 1975):
a.
Records of the review of changes in station design, procedures, and the facility license by each licensed operator or senior operator were not maintained.
b.
Records of the evaluation of the performance of licensed operators and senior operators by supervisors and/or training staff members were not maintained.
c.
Records of the lecture series and attendance for all operators and senior operators is not being documented.
These deficiencies were identified by the inspector.
