We have reviewed your letter of May 23, 1975, and the following is in response to the apparent violations respectively by alphabetical letter cited in that report. This response details each violation, corrective action, results achieved, steps taken to avoid further violations, and the date when compliance was achieved or will be achieved as applicable.

(a) Records of the review of changes in station design, procedures and the facility license by each licensed operator or senior operator were not maintained.

A " routing system" was implemented by the supervisor of plant operations on May 19, 1975. The system requires licensed operators and senior opera-tors to review items placed in the routing notebook located in the control room. All future changes or notification of changes in station design, procedure, or facility license will be placed in the routing notebook.

Adequate documentation of operator review is maintained with this system.

The system is de<:igned to operate on a continuing basis to avoid further violations. Full compliance has been achieved at this time.

(b) Records of the evaluation of the perfomance of licensed operators and senior operators by supervisors and/or training staff members were not maintained.
