I After assembly and testing of the fuel handling equipment supplied by Stearns-Rogers, it was discovered that the lower tube of the control rod grapple was loose. The tube mast is held in place by six 3/8 inch cap screws.

It was determined that the cap screws were not properly torqued and the nuts were not staked contrary to prescribed manufacturing procedures.

The bolt holes were found to be worn 1/16 of an inch. New holes were drilled, the bolts inserted, properly torqued and the nuts were staked.

This problem nas not been identified at any other facility using ry similar equipment. On this basis we consider this to be a breakdown
