- DAVIS-BESSE UNIT ONE NL* CLEAR PO'ER STATION SUPPLEMENTAL INFORMATION FOR LER NP-33-77-21 DATE OF EVENT: July 11, 1977 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRE' ICE: Failure of Containment Spray Pump 1-1 to meet pump head acceptance criteria.
Conditions Prior to O'currene_e_:
The plant was in Mode 4 with Power (MWT) = 0 e
and Load (M'K) = 0.
Description of Occurrence: On July 11, 1977, at 0005 hours5.787037e-5 days <br />0.00139 hours <br />8.267196e-6 weeks <br />1.9025e-6 months <br />, Operations per-

sonnel performed ST 5062.01, Containment Spray System Monthly Test, during which Containment Spray Pump 1-1 failed to meet the pump head acceptance cri-teria. The pump differential pressure _was 180.8 psid which is above the maxi-mum acceptance criteria of 179.5 psid. This placed the plant in the Action Statement of Technical Specification 3.6.2.1.

. Designation of Apparent Cause of Occurrence: After investigation, it was found that the suction pressure indicator, PI-1591 and flow transmitter, FT-1547 were out of calibration. This was the reason that the indicated pump performance data exceeded its acceptance criteria.
Analysis of Occurrenca: Since the pump would have met its required flowrate under accident conditions, there was no threat to the health and safety of the i
l public or station personnel.
Correct'ive Action: After recalibration of the pressure indicators, the surveil-lance test was performed at 1600 hours0.0185 days <br />0.444 hours <br />0.00265 weeks <br />6.088e-4 months <br /> en.Miy 12, 1977, and the pump met the acceptance criteria. This removed the plant from the Action Statement of Techni-cal Specification 3.6.2.1.
To prevent recurrence, the calibration frequency g
will be increased as necessary to insure that these instruments remain within i
their tolerances.
Failure Data: No previous similar events have occurred.
