Conditions Prior to Occurrencep The plant was.in Mode 1, with Power (MWT) = 693,.

Description of Occurrence: During a Station Rt.lew Board meeting on November 1, i

1997, it was recognized that 4. surveillance requirement of Technical Specification l

(TS) 4.5.2 was not completed. -one of the four High Pressure Injection Lines (1-2) had not been vented per Surveillance Test, ST 5051.01, "ECCS Monthly" at vent valve HP 72 since August 1, 1977. The Surveillance Test had been completed with the ex-caption of HP 72 which was removed from the procedure by modification-T-2232 on _ -

' Designation of Apparent Cause of' Occurrence: ' Personnel error is the cause of this Vent Valve. HP 72 was incorrectly _ removed from the Surveillance Test occurrence.

Analysis of Occurrence: There was no danger to the health and safety of the public t

or to station personnel. Only one of the-four High Pressure Injection (HPI) Lines-..~

was not ventied.

Also, once that.HPI Lines are filled, they. remain f411ed with the.

j pressure supplied from the Boratea Water-Storage. Tank.

Corrective Action

The higir point-at HP 72 was vented on November 1,1977 and..the.

modification T-2232 was veided.

Pipe hangers were verified to be in operable con-j dition on HPI Line 1-2 on November 8,1977 to verify no excessive pipe movements i

had occurred.

Facility Change Request 77-428 was writren on November 2, 1977 to request an improve-i ment.in the accessibility of vent valve HP 72.

l The deletion of a surveillance requirement will be discussed with the operations personnel involved.

~ Failure Data: No previous occurrences 6f a temporary modification to a procedure causing a missed surveillance test or action have occurred.
