  • DAVIS-BESSE UNIT ONE NUCLEAR POWER STATION SUPPLEMENTAL INFORMATION FOR LER NP-33-78-105 DATE OF EVENT: August 9, 1978 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: Post Accident Radiation Monitor RE 5030 Declared Inoperable on August 9,1978 Conditions Prior to Occurrence: The unit was in Mode 1, with Power (W T) = 1083, and Load (MWE) = 343 Description of Occurrence: At 0245 hours0.00284 days <br />0.0681 hours <br />4.050926e-4 weeks <br />9.32225e-5 months <br /> cu August 9, 1978, while perfor=ing Sur-veillance Test ST 5099.05, "Shif t Channel Check of the Radiation Monitoring System",

operations personnel discovered that the pump for RE 5030 was inoperable. This placed the unit in the action statement of Technical Specification 3.3.3.6 which requires the post accident monitoring instrumentation channels to be operable while the unit is in Modes 1, 2 or 3. This action str.tement requires the inoperable in-strument to be restored within 30 days or the unit must be placed in Hot Shutdown s within the next twelve hours.

Designation of Annarent Cause of Occurrence: The cause of the occurrence on August 9 was attributed to a component failure of the motor en the pu=p. These motors have been drawing excessive current and failing at the high ambient temperature.

Analysis of Occurrence: There was no danger to the health and safety of the public or to unit personnel. This instrument does not control any equipnent but is used for monitoring purposes only. No other systems were affected by this occurrence.

No incident requiring use of this instrumentation occurred during the ti=e the monitor was inoperable. The other containment post-accidant radiation =onitor, RE 5029 was operable during the period that RE 5030 was inoperable.

Corrective Action: Under Maintenance Work Order 78-2006, the otor was replaced on the post-accident radiation monitor RE 5030. On August 11, 1978, at 2350 hours0.0272 days <br />0.653 hours <br />0.00389 weeks <br />8.94175e-4 months <br />, Surveillance Test ST 5032.01, " Radiation Monitor Functional Test" was completed, and the post-accident radiation monitor declared operable, re=oving the unit from the action statement of Technical Specification 3.3.3.6.

Facility Change Request 78-384 has been issued to request an engineering evaluation of motor related problems on radiation monitors. Facility Change Recuest 73-299 which requestad that a cooling system for RE 5030 and RE 5029 be installed, was also preoared.
