Dear Mr. Fiorelli:
Toledo Edison acknowledges receipt of your November 22, 1976 letter and enclosed Report of Operations Inspection No. 050-346/76-20 covering the site inspection of October 4-6, 1976 and October 27-29, 1976. This report contains an apparent noncompliance with NRC requirements, listed as an " Infraction" under the heading " Enforcement Action".
I The following discussion under each of the two items involved with the infraction describes the actions we have taken including corrective action.
i Item A: " Insulation was being insta11ed'on reactor coolant system piping, on October 9, 1976, without cleaning the pipe to Class D clean 11-ness as called for in B&W Construction Company Quality Control Procedure 9A-128 (Revision 1, Item 4.4)."
As :liscussed in the Inspector's Report Details, on October 11, 1976, the requirement that pipe i.e cleaned to the specified cleanliness class, with j
verification by QC personnel, was imposed on the insulation contractor for remaining work. The boundaries of the previously installed areas were identified and a random sampit g program initiated to remove portions of the installed mirror insulation to verify pipe cleanliness conditions.
As of December 15, 1976, this random sampling program was completed cover-ing approximately 1% of the insulation installed prior to October 11, 1976.
'As a result of finding scue granular material on.the pipe in horizontal run locations d'uring this inspection, the decision has been made to remove and clean the horizontal piping runs covered by mirror insulation prior to October 11, 1976. Cleaning and reinstallation of insulation is expected i
to be complete by December 31, 1976, and in any case will be accomplished prior to fuel loading.

Page 2 December 15, 1976 Item B: " Flanges were removed from the suction lines in the emergency sump on October 15, 1976 without appropriate authorization of a Construc-tion Work Permit resulting, due to concurrent preoperational valve testing, in the discharge of approximately 40,000 gallons of water in the containment."
Thiately following this occurrence, the Construction Work Permit proce-dinees were reviewed with the Construction Manager including the degree of commpliance with the procedures. This occurrence was determined to be an imelated case of noncompliance with procedures, however, on November 17, 1976 d
all contractors were formally notified of the importance of following Startup
- Procedure 10-A when doing work on systems and equipment that have bet:n turned aver for operation.
Procedure 10-A was revised on November 20, 1976 to give tighcar control of work being done under Construction Work Permits.
