IDENTIFICATION OF OCCURRE!!CE: Emergency Ventilation System (EVS) Fans 1-1 and 1-2 Inlet Balancing Dampars Closed Cond ition.4 Prior to Occurrence: The unit was in Mode 3, with Potaer (MCE) = 0, and Load (MWE) = 0.

- Description of occurrence: At 1500 hours0.0174 days <br />0.417 hours <br />0.00248 weeks <br />5.7075e-4 months <br /> on January 5,1978, the inlet balancing dampers-to EVS Fans 1-1 and 1-2 (CV 69 and CV 73) were found closed.

Desinnation of Apparent Cause of Occurrence: Even though the exact cause is not known, the dampers were procaoly closed by some unknown person.

Since the lock-nuts that hold the damper in position were tight and the dampers could not be re-opened without looser.ing the locknuts, there is little possibility that thcoe

~ dampers vibrated shut.

Analysis of Gecurrence:.There was no danger to the health and safety of the public or to unit personnel. No incident. requiring EVS occurred during taa time that the

' dampers were closed.

Corrective Action: The dampers were returned to the balance position and the locknut tightened. These darpers are checked as part of a monthly preventive maintenance schedule, andiclosure of these dampers would show'up on the monthly surveillance test as a;zero or extremely low flow condition.

This incident will be distributed to Maintenance ani Inntrunent and Control

- personnel for raquired reading.
