Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE:

Component Cooling Water Heat Exchanger 1-1 Outlet Isolation Valve SW L424 was inoperable.

Conditions Prior to Occurrence: The unit was in Mode 4, with Power (W.CO = 0, and Load (MWE) = 0.

Descriotion of Occurrenae: At 1930 hours0.0223 days <br />0.536 hours <br />0.00319 weeks <br />7.34365e-4 months <br /> on January 12, 1978, a Reactor Operator found that SW L424 was not operating properly - it sometimes stuck open and some-times stuck closed.

Component Cooling Water Heat Exchanger 1-1 Outlet Isolacion Valve SW 1424 was declared inoperable.

This placed the unit in the Action State-ment of Technical Specification 3.7.3.1 which requires two independent operable component cooling water loops in Modes 1, 2, 3 and 4

-- Designation of Accarent Cause of Occurrence:

The cause of this occurrence is attri-uted to component failure. A solenoid valve in the control scheme was. found burned

- out.

Analysis of Occurrence: There was no danger to the health and safety of the public or to unit personnel.

The other two Component Cooling Water Heat Exchangers were operable during the time that SW 1424 was inoperable. Also, SW 1424 was placed in the fail-safe position (open).

Corrective Action

The unit was removed from the Action Statement of Technical Specification 3.7.3.1 on January 13, 1978 at 1325 hours0.0153 days <br />0.368 hours <br />0.00219 weeks <br />5.041625e-4 months <br /> when Component Cooling Water Pump 1-1 was stopped and 1-3 started.

Component Cooling Water Pump 1-2 remained in operation.

The unit thea had two operable cooling water loops.

The defective solenoid was removed, but no replacement was available on site.

Another solenoid was ordered from the valve vendor, Hammel-Dahl.

This will be installed upon its arrival which is estimated to be January 24, 1978.

Failure Data: On Decer.ber 12, 1977, Service Water Outlet Isolation Valve in Com-ponent Cooling Water Loop 1-2 Heat Exchanger was found to be closed.

The cause of the December event, however, was personnel error.
