SUPPLEMENTAL INFORMATION FOR LER NP-33-78-93 DATE OF OCCURRENCE: July 13, 1978 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: Failure to perform ST 5099.08, " Miscellaneous Valves Quarterly Test", and ST 5099.11, " Operational Checks of Passive Valves" Conditions Prior to Occurrence: The unit was in Mode 5, with Power (MWT) = 0, and Load (MWE) = 0.

Description of Occurrence: On July 12, 1978, at 1300 hours0.015 days <br />0.361 hours <br />0.00215 weeks <br />4.9465e-4 months <br />, it was discovered that Surveillance Tests, ST 5099.08, " Miscellaneous Valve Quarterly Test", and ST 5099.11, " Operational Checks of Passive Valves" were not being performed as required by Technical Specification 4.0.3.

This occurrence was discovered by surveillance test scheduling personnel while verifying that all surveillance tests are included in the scheduling program.

Since both of these tests were not required until January 1, 1978, ST 5099.08 should have been performed twice and ST 5099.11 should have been performed each time a passive valve was operated.

O Designation of Apuarent Cause of Occurrence: These tests were never incorporated in the surveillance test scheduling program, and therefore were not included in the computer schedule and the required test lists.

Analysis of Occurrence: There was no danger to the health and safety of the public j

or to unit personnel. Surveillance Test ST 5099.08 was performed on July 27, 1978 and passed the acceptance criteria. Therefore, we believe that the valves were operable' over the time period that the surveillance test was not perfor=ed.

Sur-veillance Test ST 5099.11 is only used to record the times that a passive valve is operated and is not to verify any safety related operation.

Corrective Action

On July 27,1978, ST 5099.08 was incorporated in the computer schedule and was performed satisfactorily. The passive valves in ST 5099.11 have been identified and the operational log for these passive valves is being used at the present time.

Failure Data: There have been no 'srevious similar occurrences.
