.DATE OF EVENT: July 19, 1978 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: Failure to perform required surveillance testing on Diesel Generators Conditions Prior to Occurrence: The unit was in Mode 3, with Power (MWT) = 0, and Load (MWE) = 0.

Description of Occurrence: During an NRC on site audit on July 19, 1978, it was discovered that Surveillance Requirement 4.8.1.1.2a4 was not adequately verified by ST 5081.01, " Diesel Generator Monthly Test".

The surveillance requirement specifies verification that ene diesel starts from ambient condition and accelerates to at least 900 rpm in 3.10 seconds at least once every 31 days on a i

staggered test basis. The diesel was started and tested by the surveillance test but the time to 900 rpm was not measured by the test.

Designation of Apparent Cause of Occurrence: The cause of this occurrence was a procedure inadequacy in ST 5081.01, " Diesel Generator Monthly Test".

The proce-dure referenced the Technical Specification requirement, but the measurement of t

acceleration time to 900 rpm was omitted.

Analysis of Occurrence: There was no danger to the health and safety of the public or to unit personnel. The diesels start and accelerate to 900 rpm typi-cally in less than 5 seconds. The intent of the surveillance requirements is to measure the time to insure that the diesel is up to speed and' ready to be syn-chronized. Had emergency power been required, the diesels would have been able to start and supply electrical energy to the essential busses as verified by the monthly tests.

Corrective Action

A modification (T-3029) was written to ST 5081.01, and per-formed at 1915 hours0.0222 days <br />0.532 hours <br />0.00317 weeks <br />7.286575e-4 months <br /> on July 20, 1978, which verified the surveillance require-ments. This test will now measure and record the time required by the diesel to reach 900 rpm.

Failure Data: One previous failure to perform surveillance requirements on the diesels occurred on November 30, 1977, as reported in Licensee Event Report NP-33-77-101.
