Gentlemen:
Subject: Inspection Report NO. 50-320/89-09
A special, announced, inspection was conducted by S. Sherbini of this office on
September 28 and October 12 and 13, 1989. The purpose of the inspection was to
l
review the circumstances, evaluations, and corrective actions associated with
.the apparent exposure to radiation of two workers in excess of the limits
specified in 10 CFR Part 20. A discussion of our findings was held with you and
members' of your staff by Dr. Sherbini at the conclusion of the inspection.
Areas examined during this inspection are described in the NRC Region I
-inspection report which is enclosed. Within these areas, the inspection
consisted of selective examination of procedures and representative records,
interviews with personnel and observations of the inspector.
Within the scope of this inspection, two apparent violations were identified.

One of the apparent violations stems from the exposure of two workers in excess

of the. limits for exposure to the hands as specified in 10 CFR Part 20.101. The

second apparent violation stems from the delay in reporting the exposure
incident to the NRC under the 24-hour notification requirement for the dose

as specified in 10 CFR Part 20.403(b .
