. Power Plant.. At tle conclusion of the examination, any generic findings that' evolved as a result of the examinations were discussed withithose members'of

'ywr staff identified in the enclosed report.

L As a result of this evaluation of your requalification program, it has been assigned an overall program rating of unsatisfactory for the following reasons in accordance with the criteria of NUREG-1021, ES601:

U.

Contrary to the 75% passing requirements of ES601, only 58%.of the ioperators passed the examinations.

At least 50% of the operators examined failed' common JPM questions on six

~

y separate occasions.

5 The NRC considers the facility requalification training staff deficient in the development of requalification examinations in accordance with

.ES 601 requirements.

For those individuals with unsatisfactory results, the facility should take 1

corrective action as required by its NRC approved requalification program.

It is our understanding that these individuals will not be returned to licensed duties until they are reexamined by the NRC.

To assure continued safe plant operations, the NRC evaluated two additional crews'nn the simulator on September 19, 1989. The results were satisfactory.

l NRC management met with your representatives on October 4, 1989, to discuss your ewn findings, your plans to conduct programmatic improvements, individual upgrading, and their respective completion or implementation dates. The details of these commitments are contained in Confirmatory Action Letter RIII 80021 L
