Dear Mr. Sylvia:
This letter confims the telephone conversation on July 15, 1988, between E. 6. Greenman'of this office and you regarding the events surrounding the breaking of a, compression' fitting on an instrument line in the reactor water cleanup system on July 13, 1988.'.This led to the loss of several hundred gallons of reactor water into the reactor building and is a repeat of a r

similar. event in the same line on May 28, 1988.
With regard to the matters discussed, we underst6nd that you will:-
j 1.
Determine the. root cause'of the compression fitting failures l
. including.the pressure transients wnich apparently contributed to t
both the May.28 and July 13, 1988 events.
2.
Datemine, based on the results of item 1 above, whether any generic

problems exist with the design and configuration of the RWCU system, safety related systems, or important to safa+y systems using compression fittings.
l 3.
Provide the basis for not previously checking this failed compression fitting and the other fittings in this line after the

1 first failure was detected in Mey 1988.
4.
Establish a program by July 22, 1988, to check.the adequacy of installation of conpression fittings in all safety related, and important to safety high energy lines with compression fittings to ensure that loss.of inventory and attendant contamination problems do not recur, 5.
Except as dictated by plant and personnel safety, maintain a sufficient sample size of compression fittings in the affected and still isolated RWCU line in the as-found condition to pemit subsequent evaluation.
Advise the resident inspectors prior to making any adjustments on any other fittings which may be found defective. Such notification should provide sufficient time for the inspectors to be able to observe corrective activities if desired. I QC inspection is conducted on all defective fittings found, and assure that definitive criteria are established for prtper installation.
6.
In completing your startup test program, give consideration to the effect that testing may have on similar fittings in other areas.
This would include, for excmple, testing involving fast closure of a valve or valves which could result in a pressure transient.
'7.
Maintain complete documentary evidence of the failures for NRC inspection.
i 8.
Submit to the Office of Nuclear Reactor Regulation and to NRC Region III a formal report of the stttus of your findings, conclusions, and corrective actions within 30 days of the etceipt of this letter.
1 Issuance of this Confirmatory /.ction Letter does not preclude the issuance of an Order requiring in.plementation of the above conar,itments.
None of the actions specified herein should be construed to take preced' ence over any action which you believe may be necessary to ensure plant and personnel safety.
Please let us know imediately if your understanding differs from that set out above.
