This refers to the routine safety inspection conducted by F. A. Maura,

V. P. Lougheed, and M. R. Merholz of this office on June 27 through October 23,

1989, of activities at Perry Nuclear Power Plant, Unit 1, authorized by NRC

,

Operating License No NPF-58, and to the discussion of our findings with

Mr. V. Concel and other members of your staff at the conclusion of the

inspection.

The purpose o'f this inspection was to review and witness activities dealing

l

with the performance of your first periodic containment integrated leak rate

,

i-

test and containment penetration leak rate testing, including the problems

'

experienced at Perry with excessive leakage of the MSIVs.

Within these areas,

t

the inspection consisted of a selective examination of procedures and

representative records, observations, and interviews with personnel.

'

During this inspection, certain of your activities appeared to be in violation

of NRC requirements, as specified in the enclosed Notice.

A written response

is required.

Your responsa to Item A of the enclosed notice should address

i

all gasketed containment penetrations not presently being Type B tested. We

!

also request that you respond to the Open Items identified in Paragraph 2

i

(MSIV condition trendinC), 4.c (UFSAR revision), and 6.b (Type 8 and C testing

[

pressures) of the attached Inspection Report,
