Subject: NRC Region ! Inspection Report No. 50-271/89-17

This refers to the routine resident safety inspection conducted by Mr. G. Grant

of this office on September 6 - October 16, 1989, at the Vermont Yankee Nuclear

Power Station, Vernon, Vermont.

The inspection consisted of observation of

activities, interviews with personnel and document reviews.

The results of the

inspection are described in the NRC Region I Inspection Report enclosed with

this letter and were discussed with Mr. R. Wanczyk of your staff at the con-

clusion of the inspection.

,

Your attention is directed to recent instancv, of degraded or defeated high

radiation area locked doors.

Notwithstanding extensive investigation and com-

prehensive corrective actions for previous similar events in 1988, it appears

that plant management has not been fully effective in ensuring plant staff ad-

here to procedures and policy regarding control of these areas. Although it is

recognized that the high radiation areas involved in the recent instances are

not required by Technical Specifications to be locked closed, it is essential

that your staff cnmply with all administrative controls which enhance radiation

protection. Additionally, in April 1989 two doors to locked high radiation

areas were found to be improperly secured. In view of these repeated events,

you are requested to respend in writing to this office within forty-five days

of receipt of this letter detailing your assessment and additional corrective

.

actions with respect to this issue.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of

this letter and the enclosures will be placed in the NRC Public Document Room.
