YELLOW CREEK NUCLEAR PLANT UNITS 1 AND 2 RESPONSE TO DEFICIENCY 566/80-07-01 AND 567/80-07-01 Deficiency As required by Criterion XVII of Appendix B to 10 CFR 50, and implemented by PSAR Section 17.lA.17, " Sufficient records shall be maintained to furnish evidence of activities affecting quality."
Contrary to the above, records generated by the batch plant are inaccurate on pour number Al-Pr and Al-J1. On pour number Al-Pr, records produced by the batch plant recorder are inaccurate in that batch times are unreadable, curves showing batch ingredient quantities begin several increments above the zero reference line. On pour number Al-J1 the recorded water contents for test number 1302 and 1304 are not compatible with the recorded slump tests. .
Corrective Action TVA's review of the recorder charts for contrete pours Al-P3 and Al-J1 showed that the date and time printed by the automatic printer were illegible for a significant part of the shift. However, the date and approximate time could be determined from the mixing plant records. The problem was due to dirty filters on purge air blowers and a faulty solenoid valve on the printer.
TVA's corrective action was to replace the filters and clean the solenoid valve. We are now getting clear printing.
The scale lines are printed onto the chart paper by an automatic printer during operation. The printer rolls have become worn and cause faint lines.
At present, the quantities can be determined by interpolation. New printer rolls have been ordered and should eliminate the problem completely.
The rezeroing problem has been investigated by TVA instrumentation engineers and is primarily restricted to the water, AEA, and WRA. The ingredients are released into the mixer slowly, and the recorder pins sometimes fail to rezero because of excessive pin pressure and the loss of pin momentum.
The heavy pin pressure was to keep the pins from " jumping" off the chart 4
because of plant vibration and was apparently excessive.
The failure to rezero causes only an apparent error and is being investigated further. TVA's corrective action was to optimize the pin pressure and instruct operators and inspectors to monitor and rezero if required.
The recorded water contents for test numbers 1302 r.:.o 1304 do not correlate with the slump test for concrete pour Al-J1. At tu; *tme of this pour, the procedure was to take one aggregate moisture content test per shif t and was in compliance with General Construction Specification G-2 and QCI 204 RS. l A close correlation between slump and water content would not be obtained from one aggregate moisture test per shift. The change in moisture content

of the aggregate causes a change in the water required to maintain correct slump. ,

Steps Taken to Avoid Further Recurrence .
Proper preventive maintenance will be performed to prevent a recurrence in the lack of quality in printing of records produced by the batch I;, plant recorder.
To prevent a recurrence of the rezeroing problem, TVA will investigate the plant for any excessive vibration and monitor the pin closely to k' ep the pressure optimized.
General Construction Specification QCI 204 R6 requires a moisture content
. test every two hours,'and a closer correlation will be obtained.
Date of Full Compliance Yellow Creek Nuclear Plant will be in full compliance by July 8, 1980.
