J This report is submitted to provide details of a situation reported to you by Telex on May 9,1980.

The situation 'nvolves the use of an earlier revision of amplified j

response spectra in the design of two safety related tanks on the subject project.

Details as required by 10CFR21, paragraph 21.21 (b) (3) are contained in the attached report.

It can be noted in these details that the situation has been evaluated as not a defect as defined in paragraph 21.3 (d).

This situation was initially known to the NRC during an LCVIP I v ec-ion of our Philadelphia operation during May 25 to 29, 1979.

30 South 17th Street Fhiladelphia, PA 19101 IDENTIFICATION OF THE FACILITY, THE ACTIVITY, OR THE BASIC COMPONENT SUPPLIED FOR SUCH FACILITY OR SUCH ACTIVITY WITHIN THE UNITED STATES WHICH FAILS TO COMPLY OR CONTAINS A DEFECT.

Washington Public Power Supply System (WPPSS), Units WNP-1 & WNP-4; the design, fabrication, and erection of the Borated Water Storage (BWST) and the Demineralized Water Storage (DWST) Tanks.

IDENTIFICATION OF THE FIRM CONSTRUCTING THE FACILITY OR SUPPLYING THE BASIC COMPONENT WHICH FAILS TO COMPLY OR CONTAINS A DEFECT.

Welk Brothers' Metal Products, Inc., P. O. Box 19099, Spokane, WA 99219, is the firm supplying the tanks.

Design parameters, such as seismic response input, have been supplied by UE&C.

Based upon the design parameters, the vendor is responsible for design, fabrication, and erection of the tanks at the job sit'.

At the time of identification of the deviation, the tank shell plates had been shipped to the job site, but had not been ?rected.

l NATURE OF THE DEFECT OR FAILURE TO COMPLY AND THE SAFETY HAZARD WHlCH IS CREATED OR COULD BE CREATED BY SUCH DEFECT.0R FAILURE TO COMPLY.

Refer to USNRC, Region IV, letter dated June 14, 1979, Notice of Deviation and Section 11.8.3 for details of the potential defect.

This deviation could potentially have resulted in a loss of safety function of the subject tanks.

i Evaluation of this deviation revealed that a defect, as defined in 10CFR21, did not exist; hence a substantial safety hazard could not have been created.

THE DATE ON WHICH THE INFORMATION OF SUCH OEFECT OR FAILURE TO COMPLY WAS OBTAINED.

USNRC QA Program Inspection conducted during the time period May 21 - 25,1979.

IN THE CASE OF A BASIC COMPONENT WHICH CONTAINS A DEFECT OR FAILS TO COMPLY, 2

THE NUMBER AND LOCATION OF ALL SUCH COMPONENTS IN USE AT, SUPPLIED FOR, OR' BEING SUPPLIED FOR ONE OR MORE FACILITIES OR ACTIVITIES SUBJECT TO THE REGULATIONS IN THIS PART.

l Not applicable; the poteatial defect concerned the unique application of a seismic response spectra to a unique set 4

The corrective actions.aken are complete and did not result in i

i identification of any defect or substantial safety hazard in the design of the Borated Water Storage or Demineralized Water Storage Tanks..The proper seismic response spectras.for the BWST and DWST i

tanks were incorporated into Specification 243, the vendor has re-analyzed the tank seismic response, and UE&C has reviewed the analysis.

No changes were required to the tank design, fabrication or erection.

- The revised seismic response spectra was issued to the Contractor, by Change Order, on July 23, 1979.

The vendor submitted an addendum to existing stress reports for the BWST and DWST tanks on September 28,.

1979 confirming that the net seismic response of the tanks is slightly i

less than was originally calculated, and that no changes to the tanks I

were required.

ANY ADVICE RELATED TO THE DEFECT OR FAILURE TO COMPLY ABOUT THE FACILITY,

ACTIVITY, OR BASIC COMPONENT THAT HAS REEN, IS BEING, OR WILL BE GIVEN TO PURCHASERS OR LICENSEES.

This refers to the Q' Program Inspection conducted by Mr. J. R. Costello of this A

office on May 21-25, 1979, of your facility at Philadelphia, Pennsylvania, and to the discussions of our findings with Mr. R. J. Vurpillat and members of your staff qt the conclusion of the inspection.

Areas examined during the QA progra= inspection and our findings are discussed in the enclosed report. Within these areas, the inspection consisted of an examination of procedures and representative records, interviews with personnel, and observations by the inspector.

During this inspection it was found that the icple:entation oi your QA program failed to meet certain NRC requirements.

The specific findings and references to the pertinent requirements are identified in the enclosures to this letter.

Please provide us within thirty (30) days a written statement containing, (1) a description of steps that have been or vill be taken to correct these items,

(2) a description of steps that have been or vill be taken to prevent recurrence, and (3) the dates your corrective actions and preventive measures were or vill be completed.

In accordance with Section 2.790 of the Coc=1ssion's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations, a copy of this letter with enclosure and your reply together with the enclosed inspection report vill be placed in the Commission's Public Document Room.

If this report contains any infor=ation that you believe to be proprietary, it is necessary that you =ake a written application within thirty (30) days to this office to withhold such informatio'n from public disclosure.

Any such application cust include a full statement of the reasons on the basis of which it is claimed that the infor=ation is proprietary, and should be prepared so that proprietary infor=ation identified in the application is contained in a separate part of the documma;.

If we do not hear from you in this regard within the specified period, the report vill be placed in the Public Docu=ent Room.
