Dear Mr. Grier:
SNUPPS Project, Bechtel Job No. 10466 Report of Defective Components Under 10CFR Part 21 In accordance with the requirements of 10CFR Part 21, this is to confirm reporting of a 10CFR Part 21 deficiency, by telephone on 5/14/80, by Bechtel personnel to Mr. L. Narrows, Office of Inspection and Enforcement, R'egion I, U. S. Nuclear Regulatory Commission. The report concerns pipe support sway struts with loose bushings furnished to the Callaway Unit 1 and Wolf Creek Jobsites by Corner & Lada Co., Inc., Cranston, Rhode Island.
The same deficiency was reported on 5/12/80, to Mr. R. McGaughy, Office of Inspection and Enforcement, Region I, U. S. Nuclear Regulatory Com=ission, by SNUPPS as a generic Significant Reportable Deficiency per 10CFR50.55(e).
This report is being submitted by Bechtel Power Corporation, per 10CFR Part 21, since Bechtel's evaluation indicates that it is reportable to the NRC per 10CFR Part 21 requirements. Corner & Lada was advised, verbally and in writing, by Bechtel to report the deficiency to the NRC. However, Corner &
Lada has advised Bechtel, by a TWX dated 5/14/80, that they are not required to file a report under 10CFR Part 21 since they have no prior knowledge of any struts manufactured with loose bushings (bearings) and have found no deficiencies in their design criteria and manufacb; ring processes which would create the locse bushing (bearing) condition.
Therefore, Bechtel is filing this report, is required by 10CFR Part 21 requirements.

, Following is specific information required by 10CFR Part 21:
1.
INDIVIDUAL REPORTING Bernard L. Meyers SNUPPS Project Manager Bechtel Power Corporation 15740 Shady Grove Road Gaithersburg, Maryland 20760 2.
FACILITY AND COMPONENT IDENTIFICATION Callaway Unit 1 (Union Electric Company) Fulton, Missouri and Wolf Creek (Kansas Gas and Electric Company) Burlington, Kansas.
Approximately 1300 of Size 2 and 3 sway struts delivered to the jobsites prior to 11/28/79.

SUPPLYING FIRM Corner & Lada Co., Inc.

NATURE OF DEFECT Loose bushings in size 2 and 3 sway struts at the clamp end. As a result of the loose bushings, the sway strut paddle can become completely disengaged from the bushing, causing a 3/8" gap between the innersurface of the paddle and the pin. Existence of the gap, during a seismic event, cay result in piping and/or piping welds being overstressed causing a possible safety hazard.

DATE OF NOTIFICATION OF THE DEFECT Bechtel Power Corporation was notified verbally, on 5/8/80, by Corner &
Lada that the tests, conducted by them as requested by Bechtel, to deternine if the paddle with loose bushings on the clamp end for Size 2 and 3 sway struts can become completely disengaged indicated that this possibility exists.
Bechtel Power Corporation Mr. Boyce H. Grier Pby 19, 1980 On 5/12/80, Bechtel concluded that, based on Corner & Lada's test results and since sway struts with loose bushings had been shipped to the jobsites and accepted for use, this defect was reportable to the NRC per 10CFR Part 21.

OTHER COMPONENTS WITH IDENTICAL DEFECT None known to Bechtel at this time.
Corner & Lada should be contacted, by the NRC, for identification of other facilities to which similar components may have been supplied.

CORRECTIVE ACTION All Size 2 and 3 sway struts, delivered prior to 11/28/79 had been inspected and any bushings found loose had been reworked (" staked") by Corner & Lada personnel at the Callaway Jobsite between 1/21/80 and 2/15/80.
Similarly, all Size 2 and 3 sway struts, delivered prior to 11/28/79, had been inspected and any bushings found loose had been reworked (" staked") by Corner & Lada personnel at the Wolf Creek Jobsite between 11/30/79 and 2/20/80.
8.
NOTIFICATION TO OTHER LICENSEES The SNUPPS organization, Union Electric Conpany and Kansas Gas and Electric Company have been notified.
