Immediate Report of Reportable Occurrence This letter is to confirm the prompt report made to Mr. M. H. Malmros, Resident Inspector by Mr. W. S. Orser, Manager, Operations and Maintenance, Trojan Nuclear Plant concerning a situation where it could not be assured that containment integrity was maintained during refueling operations. During the period of core alteration, a main steam isolation valve was opened for inspection while a steam generator secondary manway was removed.
This occurrence will be reported to the Nuclear Regulatory Cornission as a Licensee Event Report in accordance with our established Technical Specifi-cations.
