O APPENDIX A University of Arizona College of Engineering Tuscon, Arizona 85721 Docket No. 50-113 License No. R-52 NOTICE OF VIOLATION Based on the results of an NRC inspection conducted on fiarch 18 and 19, 1980, it appears that certain of your activities were not conducted in full compliance with conditions of your NRC Facility License No. R-52, as indi-cated below.
1.
Technical Specification 3.6 states in part:
"The reactor shall not be operated unless the facility ventilation system is in operation except for periods of time not to exceed two days to pennit repairs to the system.
During such periods of repair:
a.
The reactor shall not'be operated at power levels above 10 kw;..."
Contrary to the above, on or about April 23, 1979, the reactor was operated for a period of about six hours at power levels above 10 kw with the facility ventilation system not in operation.
This is a deficiency.
2.
Technical Specification 6.2.a states in part, that "There shall be a Reactor Committee..." and Technical Specification.6.2.f states, "The committee shall meet at least quarterly."
Contrary to the above, the Reactor Comittee failed to meet between May 11 and November 8, 1979 which is in excess of three months and ex-cludes the July to September 1979 calendar quarter.
This is a deficiency.
