l APPENDIX A NOTICE OF VIOLATION Baltimore Gas and Electric Company Docket Nos. 50-317 50-318 Based upon the results of an NRC Inspection conducted on December 26, 1979 to January 31, 1980, it appears that certain of your activities were not conducted in full compliance with NRC regulations and your facility Licenses Nos. DPR-53 and DPR-69.

These items are infractions.

Item B applies only to DPR-53.

10 CFR 50, Appendix B Criterion XVI, states in part:

"Mee ures shall be established to assure that conditions adverse to quality, such as failures, malfunctions, deficiencies, deviations, defective material ar.d equipment, and nonconformances are promptly identified and corrected.

In the case of significant conditions adverse to quality, the measures shall assure that the cause of the condition is determined and ccrrective action taken to preclude repetition."

Criterion V, Appendix B, 10 CFR 50, states:

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.

Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

The Baltimore Gas and Electric Company Quality Assurance Manual for Nuclear Power Plants states in Part 16, Corrective Action:

"Nonconformances are eval-uated to determine the need for corrective action under the control of estab-lished procedures... Conditions adverse to quality that require corrective action may be identified by... observation of personnel involved in activities at the Calvert Cliffs Nuclear Power Plant...to preclude the recurrence of conditions adverse to plant safety, prompt corrective action is initiated when a nonconformance is identified."

Contrary to the above, conditions adverse to quality were not promptly corrected at the Calvert Cliffs Nuclear Power Plants, and procedures for ensuring that conditions adverse to quality are promptly identified were not prescribed; in that a ground condition developed on vital direct cur-rent circuitry on or before November 5, 1979 with effective corrective action to clear the ground (s) not accomplished until after unplanned Reactor Coolant Pump 12B operation occurred on January 29, 1980 because of multiple grounds on that circuitry; and in that there are no procedural controls which require ground readings to be regularly taken and reviewed, and no minimum ground resistance requirements are specified.

Contrary to the above on January 29, 1980 when the Power Operated Relief 1

Valve opened, apparently inidvertently at 11:45 a.m., no action was taken l

to investigate the cause of the occurrence until the NRC Resident Inspector asked about the cause on the morning of January 30, 1980.
