O Public Service Company of Colorado Fort St. Vrain Docket No. 50-267/80-06 NOTICE OF VIOLATION Based on the results of an NRC inspection conducte(. March 3, 1980 to March 28, 1980, it appears that certain of your accivities were not conducted in full compliance with NRC requirement, as indicated below:
1.
Technical Specification AC 7.4 states in part, " Written procedures shall be established, implemented and maintas ned covering activities
. surveillance and test activities.
Administrative Procedure (ADM) No. 14, Calibration of Plant a.
Instrumentation and Test Standards, states in part, " Complete calibration records of each transfer device shall be maintained in the departmental files."
Contrary to the above, calibration records were not maintained for transfer devices EGG 992 or EGG 440.
b.
Surveillance Procedure SR 5.4.1.1.6c-R specifies that an EGG 992 is to be used when performing a Primary Coolant Moisture Scram Calibration.
Contrary to the above, the surveillance performed in accordance with SR 5.4.1.1.6c-R on November 14, 1979, utilized an EGG 440.
This is an infraction.

Technical Specification LCO 4.1.9, Core Temperature Rise, states that, "Whenever the reactor is pressurized to more than 50 psig and the core inlet orifice valves are set at any positions other than for equal flows, the measured helium coolant temperature rise through any core region shall not exceed the limits given in Figure 4.1.9-2 (at the appropriate power level)."
Contrary to the above, the limits imposed by Figure 4.1.9-2 for measured coolant temperature core region rise was exceeded for several regions on March 11, 1980 beginning at approximately 9:52 p.m. and continuing on March 12, 1980 until 12:41 p.m at which time all temperatures were returned within required limits. Additionally, on March 12, 1980, all region temperature rises, at times, exceeded the allowable rise for that specific power level at which the reactor was operating.
At the time the limits were first exceeded, reactor pressure was 366 psia and reactor power was 0.9%.
During the time period mentioned (3/11/80-3/12/80) reactor power varied between approximately 0.05% to 2.0%.
This is an infraction.
