TOLEDO EDISON COMPANY DAVIS-BESSE NUCLEAR POWER STATION UNIT ONE SUPPLEMENTAL INFORMATION FOR LER NP-33-80-46 PATE OF EVENT: April 18, 1980 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: Steam Generator Level Transmitter LTSP9A6 and LTSP9B6 to Steam and Feedwater Rupture Control System (SFRCS) Out of Calibration Conditions Prior to Occurrence: The unit was in Mode 5, with Power (MRT) = 0 and Load (Gross MRE) = 0.

Description of Occurrence: On' April 18, 1980 at 1500 hours0.0174 days <br />0.417 hours <br />0.00248 weeks <br />5.7075e-4 months <br /> during Steam Generator Level to SFRCS Refueling Calibration Surveillance Test ST 5031.16, Steam Generator Level Transmitter LTSP9A6 was found to be out of calibration. At an applied differ-ential pressure of 359.4 inches differential pressure, which corresponds to the low level and trip setpoint, the output from Analog Amplifier AA01 in Steam Generator Level 7

j Instrument Cabinet Channel 1 was found to be 0.8338 volts DC, whereas the maximum amount allowed was .8245 volts DC.

I Further evaluation on April 27, 1980 of the maximum allowed amount showed an error in the calculation. As a result of this evaluation, one other Steam Generator Level Transmitter LTSP9B6 was found to be out of tolerance at this specific level.

Neither occurrence placed the unit in the action statement of Technical Specification 3.3.2.2 since it required the inoperable channel to be placed in the tripped condition within one hour in Modes 1, 2, and 3 only. The unit was in Mode 5 at the time of the occurrence. ,

l Designation of Apparent Cause of Occurrence: The apparent cause of the occurrence was determined to be a zero shift on the two above mentioned instruments coupled with a tightening of tolerances as a direct result of the recent change in steam generator low level bistable setpoints on November 17, 1979, see Licensee Event Report NP-32-79-12.

Analysis of Occurrence: There was no danger to the health and safety of the public or to station personnel. Of the total of eight steam generator level transmitters to the SFRCS, two of the eight were found to be out of the allowed tolerance. The actual levels displayed based on instrument tolerance and applied differential pressure were 4

rore conservative than the levels that would have been achieved on steam generator low l

level trips prior to changes made to _ steam generator low level bistable setpoints on November 17, 1979, s'e Licensee Event Report NP-32-79-12. The results of the safety avaluation performed based'on previous steam generator low level bistable setpoints have shown no cause for a safety concern.

Failure Data: There have been no previous occurrences of calibration problems with 4

l analog amplifiers.
