O Docket No. 50-128 ENCLOSURE NO. 1 NOTICE OF VIOLATION Based on the results of the NRC inspection conducted during March 4-6, 1980.
it appears that one of your activities was in noncompliance with NRC regula-tions and conditions of your NRC License No. R-83 as indicated below:
TS 6.5, " Operating Procedures" states that written operating procedures shall be adequate to assure the safety of operation of the reactor.
SOP I-G.1 " Distribution of SOP's" requires that (1) new SOP's be approved and placed in its appropriate place, (2) that the index will be updated, and (3) that a complete up-to-date set of SOP's is required to be maintained at the Reactor Console.
Contrary to the above, Reactor operations procedures that were open and in use at the reactor console during a reactor startup on March 5 and 6, 1980 were out-of-date as compared to the Master Copy and the index had not been updated.
