During the course of the review of T!!I-2, we had expressed concern over adequate blending of sodium hydroxide and sodium thiosulphate in order to maintain acceptable pH and iodine removal capability in the reactor building spray water.
To resolve this problen, the applicant had deleted the thiosulphate completely.and piped thc Na0H so that it was injected into the B'.lST discharge upstream of the suction of both the R3 spray pumps and the safety injection pumps.
Review by appropriate branches Q
a~t he time confirmed thip would not cause a safety poblem.
P I
p::
Since the plant went crifical, two reactor ' trips and subsequent ECCS
' IT actuations have occurrm!.
(The causes of these actuations are unrel t.ed F
A and if necessary uill be' dealt with in separate correspondence as well as Other aspects of these events). PNs describing the events are attached.
i f
Both events involved injection of Na0H into the RCS; in addition, it
' was realized af ter the first event that the t;aOS contains approxinately 1%-chlorides, which resulted in chloride concentrations of about 3PPil in the RCS.
v5 Y--
llet Ed is still examiding the injection arrangement - at the moment
[
they do not consider,it a significant safety problem, but it is cer-g tainly at least an operating problem for themi Their evaluation will 22 be foniarded when e a4":d.

g Given these tuo events, please review again the safety significance y
of injection of chloride-containing MaOH into the RCS.
