paration to cold shutdown for RCP seal replacenent.
During the transitien frca turbine to turbine bypass the prinary systen experienced a pressure transient to 2267 psig. The power relief valve actuated at 2257 psig and failed to close. Tais,
caused the Quench Tank rupture disc to rupture.
Failure to prenptly close the power relief isolatica valve caused violations of the fuel compression curve, cooldown rates and RC pumps NPSE curve.
Inspection of the pct.cr relief valve ; f tc.
shutdcun indicated that the pilot valve lever had rcnained in the perted pcsiticn preventing :;he nain valve f rpn r,escatin,g. Mestraint cf the lever uns caused by 19
