Contrary to Technlent Specification 4.16 a 0.97 millicuric radiation source j

designated ONS #60 was not Icak tested prior to use when identified as being glargerthan100microcuriesofbetaactivityduring1976.

RESPONSE

The identified source was leak check'ed on September 8,1976, and the results were acceptable with less than.005 pCi leakage as required by Oconee Technical Specification 4.1.6.2.

A complete check of the source inventorv system was per_ormed to determine if any other discrepancies similar to the one identified in this inspection existed.

None were found.

Personnel that perform the source inventory and leak checks have been cautioned to double check themselves when transferring source information and to verify j

that they have leak checked all required sources.

Contrary to Appendix B Technical Specification,1.2.B. pH limits assigned for all water discharged from the plant site were exceeded on February 12, 1976 and Fbrch 16, 1976.

Although identified and reported by the licensee, the corrective actions taken to prevent recurrence were inadequate.

RESPONSE

An in-line pH monitor is presently in service on the outlet of the Oconee l

lower settling basin and provides remote readouts to the water treatment room.

i As stated in our letter of February 19, 1976 responding to the incident of February 12, 1976, similar in-line pH monitoring of the station yard drains i

l will also be installed.

This action will be completed by March 1, 1977.

To l

afford additional protection against the possibility of exceeding pH release limits, a station modification is being implemented to provide audible alarms in the Oconee control rooms in the event these in-line monitors give indications of pH releases beyond allowable Technical Specification limits.

This modifica-tion will be completed by April 1, 1976.
