99900509/78-05 NOTICE OF DEVIATION Based on the results of a NRC inspection conducted on October 17-20, 1978, it appeared that certain of your activities were not conducted in full compliance with NRC requirements as indicated below:

A.

10 CFR 50, Appendix B, Criterion V (Instructions, Procedures and Drawings) states in part, " Activities affecting quality shall be prescribed by documented instructions, procedures or drawings...

and shall be accomplished in accordance with these instructions, pro-cedures or drawings."

Stone & Webster Engineering Assurance Procedure EAP 15.2 (Handling of Nonconformance and Disposition Reports by Engineering) states in part (paragraph 3.5.1), "The N&D Coordinator shall: a. Log the necessary information in columns 6, 7 and 8 of the N&D Report Log.

Column 8 of the N&D Report Log indicates whether or not a Problem Report will be prepared as a result of the N&D Report.

Contrary to the above, the current N&D (Nonconformity & Dis)osition)

Report Log for the project, covered under J.0. Nos.12210 a,d 12330, did not have the necessary information in all cases to indicate whether or not a problem report was required.

Corrective and preventive measures were established for this deviation prior to the conclusion of the inspection and no further written response is necessary (See additional comments in DetailsSection I.C.3.a.).

B.

Stone & Webster Engineering Assurance Procedure EAP 15.2 (Handling of Nonconformance and Disposition Reports by Engineering) states in part (paragraph 3.2.2), "A technical justification shall be developed to support all dispositions of ' accept-as-is' or ' repair ' The justifi-cation shall be clearly stated and self-explanatory using additional sheets and sketches as necessary to illustrate the justification."

. Contrary to the above, technical justification was not provided on N&D Reports (1) PQC-P-0440 (Condenser L. H. and R. H. Lower Shell Stations) dispositioned accept-as-is for part of the nonconformance and repair for the other part, (2) PQC-S-2979 (Stiffener) dispositioned repair and (3) PQC-S-2992 (Reinfnrcing Bar) dispositioned accept-as-is.

Corrective and preventive measures were established for this deviation prior to the conclusion of the inspection and no further written response is necessary (See additional comments in DetailsSection I.C.3.b).
