OPERATING PLAN FOR PEACH.30TTON IN5PECTIONS OURING RE5TART POWEP. TEST!M PROOP.AM The Peach Botton Atomic Powar Station ins been shutdown since Narch 1987 to make substantial improvements to station culture and operator attitudes and to improve management.

PEco is preparing to undergo an extended power ascension test program, pending restart approval by the NRC. The Commission i s sched-91ed a meeting to vote on the restart of the station on April 17, Sg ud, if authortied to restart, the licensee intends to commence tw power ascension program immediately thereafter. Region I has recomunended that the facility be permit:.ed to restart based on an extensive inspection and review program to independently review the improvements made to the facility and its management to date.

During implementation of the power ascension prcgram for Unit 2, the NRC has committed to substanital inspection coverage of the facility, including round-the-clock inspector coverage of key safety significant plant evolutions.

Enclosed is the proposed operating plan describing the Region I augmented inspection progran:, a Division of Reactor Projects temporary organization change, and the delegation of authority for NRC oversight of thir Peach Bottom restart activities. General cuidance and specific instructions for fsplemen-tation of the augner:ted inspei:tion program are also included in this plan. The i

licensee predicts that the restart activities and the power ascension program, which includes three holdpoints requiring your release prior to proceeding, will take about four amnths to complete. The augmented ing/ection program will remain in effect until completion of the power ascension prograc.

Introduction The Regten I augmented inspection program for the Peach" Bottos Unit 2 restart effort during the power ascension testing and taittal plant operations as well as the special organization which is responsible for conducting onsite inspections of licensee restart operations durin) this period is modeled after the TMI-1 restart program of 1985 and the Pilgrim restart program of 1988.

This operating plan provides both general guidance end specific instructions for implementation of the Peach Bottom Restart $taff inspection activities.

Puroose The Peach Bottom Restart Staff was established to suustantially augment the normal NRC resident and regional inspection activities (1) to provide increased NRC awareness and understanding of the safety of peach Bottom startup operationr (2) to provide timely NRC staff response to operational problems or events that may occc-during this period, and (3) to provide a sound technical basis for deters.. :ing the effectiveness of licensee management controls for safe facility operation. All NRC staff and con-tractor personnel assigned to the Peach Botton Restart Staff should become familiar with the guidance and instructions in this plan so that these

objectives are attained.

Organization The organization of the Peach Bettom Restart Staff is depteted on the attached organization chart (Attachment 1). The Restart Staff will be headed by the Restart Nanager who will be supported by an Adeinist.'4tive Assistant.

The Restart Staff will consist of qualified insp6ctors from the peach Bottom Resident Office, the Region I Divisions of Reactor Projects, Reactor Safety, and Radiation Safety and Safeguards, the Office of Nuclear Reactor Regulation and resident inspectors free other sites. NRC contractors

will be used as needM,. The total staff size will vary dependtag on the licensee activities to be reviewed and will be organized and coordinated by the Restart Manarer. An inspection assignment schedule will be provided s

to the Restart $taff at a later date by the Restart Manager.

Infot.aation Flow Proper and effective communication of both routine and problem-related information obtained by the Peach Bottos Restart $taff personnel during their inspection activities is essentisi in assuring appropriate NRC foi owup actions and evaluations of the Power Ascension Program. Most inspection or monitoring activities conducted during the Peach Bottoa startup program will be pre planned, based on the antteipated facility operations.

In some cases, designated region-based inspectors will be dispatched to the site to observe selected startup tests or to take measure-ments during certain changes in plant operation. Therefore, any changes j

Res.tart During Power Ascension Testing to planned licensee activities or oNrational problems that could poten-ttally impact the licenstee's test schedule should be brought to the atten-tion of the Peach Bottom Restart Marager as soon as possible. Any safety concerns, apparent or potential violations of regulatory requirements, or items of stallar significance should be promptly conveyed to the Peach Botton Restart Manager after the matter has been identified to the approp-riate licensee representative (Shift Supervisor, Shift Manager or Operations Superintendent).

Events reported to the shift inspector can be classified generally into two categories:

(1) those for which an EWS call was made such as for 10 CFR 50.72 reports which include energency plan classified events, and (2) those that are regulatcry in nature (10 CFR 20,10 CFR 50.73, etc.) or for information only (such as itees of potential public interest).

Within the scope of those events that do not require an ENS call, there may well still be a safety signtficant event and thus require attention on the part of the NRC.

Safety significant events. ENS calls, and items of potential public interest will be reportec to the Peach Bottom Restart Manager as soon as reasonably pcssible.

The Peach Bottom Restart Manager will inenediately report safety significant events and ENS calls to the responsible Region ! DRP Section Chief or, if unavailable, the DRP Branch Chief or Director.

The Restart Manager will also evaluate the need to report itelns of potential public interest to the ORP Section Chief.

Licensae notification of an egent to the shift inspectors of the Restart Staff does not relieve the licensee of its reporting responsi-bilities under 10 CFR 20, 21, 50, 72 and 73.

peach Botton Restart Staff Insoections Throughout the initial period of Peach tottom restart operations, all onsite inspections will be coordinated by the Peach Gotton Restart Manager and con-ducted in accordance with an integrated inspection plan approved by the DRP Branch Chief.

These inspections inc?ude those conducted by (1) the rest-j dent inspectors. (2) the Region I-Y/NWNRC contractor shift personnel, (3) the region-based project inspectors, and (4) regional specialists.

The inspection observations, findings and enforcement actions in these areas will be documented in a consolidated inspection report. The Peach Bottom Restart Manager is responsible for preparing and maintaining current app opriate inspection plans.

The Administrative Assistant for Peach Botton Restart is responsible for coordinating preparation of the con-solidated inspection report.

In addition, periodic status reports will continue to be issued to keep NRC senior management and the public informed as to the pregress of the restart effort.

will be assigned to observe the condu:t of control room operations and licensed activities performed in certain other plant areas. Specifi-inspection/observation iten,s will be pre planned, to the extent practi-cable, in conformance with a pre-established inspection plan.

overview of the inspection coverages for key plant evolutions is shown as Attachment 2 to this plan. Each person providing shift coverage nor-mally will be assigned to work a designated 8-hour period for seven con-i secutive days (Monday through Sunday).

The typical shift inspection periods will be 7:00 am - 3:00 pm, 3:00 pm - 11:00 pm and 11:00 pm -

7:00 am, cenststent with the licensee's shif t schedule. A specialist inspector will be assigned in accordance with attachment 2 to eenitoe testing activities and to review test results during initial criticality and low power testing.

Inspection support from the regional technical divisions will be requested, scheduled and planned at the earliest possible date, commensurate with the evolution of the power ascension schedule.

prior to the first assignment on shift coverage, each shift inspector will be provided a half day period for:

site specific training and access processing, an orientation to licensee administrative procedures and con-i trols including work control procedures and the Peach Bottom daily routine, j

site organization, plant layout, plant systess and the Technical Specift-caticns, and an observation period in the control room. The site-specific training and access processing assumes the individual is certified as trained in basic radiation protection. During this orientation period, certain documentation will be available for inspector self-study and general familiarization.

Inspection Documentation All Peach Sotton Restart Staff inspection activities are intended to be conducted in accordance with a pre established inspection plan.

The inspection plan will identify specific items to be reviewed or observed by designated individuals based on the licensee's scheduled startup activ-j ities, including major system functional testing, expected changes in operating modes and planned reactor trips.

The plan will be updated daily based on anticipated near-ters restart test operations and general plant operations as detemined by Restart Staff members during licensee pre-shif t briefings, daily operations meeting, test planning asetings, Plan-of-the-Day (Tripod) meetings, or similar contacts with licensee represen-tatives.

The inspection plan will be prgparpd by the Pear.h Anttom testart Manager anc approved by the responsible DRP tranch Chief on or before the start 'of the pre-established inspection period.

Restart During Power Ascension

. Testing While assigned to shift covierage, each NRC inspector/contractor will maintain a shift Inspector's Log. The purpose of this log is to record routine inspector observations and all significant findings or potential safety concerns.

The log should indicate the disposition of identified concerns by the licensee and NRC Peach Bottos Restart Staff rept-esentatives and should identify all significant information exchanges.

Licensee documents listed in the log shou:d be identified ir. :(:fficient detail (procedure or record number, title, revision, issue date and completion date) for subse-quent reference in the inspection report.

Each inspector is responsible for preparing romplete inspection report input documentation prior to the end of his/her last day onsite during the inspection period.

The input documentation shall include a draft inspection feeder report, statistical inspection data (766) form input, SALP input (positive and negative findings), ant draft enforcement citations and outstanding items list input (if appropriate). Issues identified but not resolved by a shif t 'nspector will be passed to a cognizant staff inspector for followup as determined by the Restart Manager. The staff inspector will then bear the responsibility for preparing ti,e inspection feeder report on that issue. The Administrative Assistant for the Peach Bottom Restart Staff will coordinate final inspection report preparation, typing and management review 50 that the report is issued in a timely manner.

i The inspection report forstat and content will vary fram m6nth-to-month dependent on the nature of the licensee activities reviewed. However, the following major report sections/paragraphs may be included in the final report:

General overview/description of NRC personnel participants in the inspection, ongoing plant operations and the scope of NRC inspection activities, significant contacts with licensee representatives, significant restart silestones (including all NRC hald points) com-plated and a summary of operational events that occurred during the inspection period.

Chronological summary of plant operations.

Operational event assessments (if appropriate).

Shift operations includf ag logs and records review and facility monitoring (management planning and direction, conduct of operations and interface effectiveness both on shift and across functional organizations). Both objective and subjective observations and findings.

Surveillance testing activities.

General Information and Guidance This section contains information and guidance that describes policy, operating guidelines and routine instructions that are applicable to Peach Bottom Restart Staff practices.

t A staff briefing and planning meeting will be conducted each weekday at 7:30 am to review both recent and near-term Itcenset startup i

activities and facility operations. This meeting should be attended by all currently assigned Region I Peach Botton Restart Staff members and the off going (11:00 pm - 7:00 an) shift inspector. The on-shift inspector may attend if the individual is not following specific activities.

Immediately following this meeting, the Peach Bottom Restart l

Manager will conouet a telephone briefing with the responsible DRP l

Section Chief.

An EDO Daily Highlight Report or a Morning Report will be transmitted to Region I, as required, by 8:30 am. The off-going (11:00 pm -

7:00 an) shift inspector is responsible for preparing the first draft of these reports prior to 7:30 as.

The duty shift inspector normally is expected to remain onsite during the entire shift period. Because routine day shift activities out-side the Control Room would be covered by resident inspectors or Regional specialists, the 7:00 am - 3:00 pu shift inspector should i

remain within or nearby the Control Room. Other shift inspectors should spend their time about equally within the control room and other plant areas.

There are no specific restrictions regarding when Restart staff members may take meal breaks; however, off-site meals should be j

planned during periods of low test activity. Onsite vendor/canteen food is normally available.

All public inquires and media requests for status/interviews shouh be referred to the Peach Botton Restart Manager. All allegations should be reported promptly to the Restart Manaper and relayed to the DRP Sectice '.1ef.

Restart During Power Ascension Testing Three hold point release (startup contieustion) memoranda will be sent to the Region 1 Regional Administrator from the Director,-

DRP.

These memoranda will provide th6 justification and bases for permitting startup testing to be conducted up to the next hold point.

The criteria for each specific hold goint release will be developed by the Restart knager and formally approved by the DRP Branch Chief.

The Peach Bottom Restart knage-is responsible for coordinating the format, content and preparation for each hold point release memorandum.

A Periodic Status Report will continue to be prepared. That report will describe the major testing accomplishments, significant operational problems or events a.nd similar items of general interest.

The report preparation will be coordinated by the Peach Bottos Restart Manager.

i The Peach Bottom Restart Staff response to events/emergency situations is described in the NRC Region I Incident Response Plan with the Restart Manager fulfilitng the role cf the Senior Resident Inspector.

All Restart Staff personnel should be familiar with the general provisions and their specific emergency responsibilities as described in that plan.

As described in the plan, the Restart Manager will function as the senior NRC official onsite in the event of an emergency until properly relieved by the NRC $1te Team. The Plan will be available in the Peach Bottom Resident Offict 9.

Provisions for Observation of the Restart Staff Insoections by Representatives of the Commonwealth of Pennsylvania and the State of Marylanc The Commonwealth of Pennsylvania and the State of kryland have expressed a continuing interest in the activities at the Peach Bottom station and may request to accompany the inspectors of the Restart Staff.

Provisions for accompaniment of the Restart Staff inspectors by repretentatives of by the DRP Director and the Region I $tate Liaison Dffic e-the Regional Administrator. The Restart Staff inspectors will be briefed by the Restart $taff Manager on the formal protocols for any arrangement made with the $tates.
