This item is a deficiency.
Appendix A to 10 CFR 55 requires records to be kept for requalification pr.ograms for licensed operators of production and utilization facilities.
The Pilgrim Training Manual, Revision 3, approved May 31,1978, prescribes Form 0-3, PNPS Lic'ensed Operator Requalification Summary for a portion of

this record keepi.ng. The Manual requires posting of the results of the annual requalification examination and en annual review of the traini.ng record of each licensed operator.
Contrary to the above, the 1979 examination results and the station manager's review were not documented on the Forms 0-3 reviewed for ten licensed operators, although ample evidence of the administration and analysis of the annual examination and personnel evaluation was available.
