' d Arkansas Power and Light Company Arkansas Nuclear One, Unit No. 2 Docket No. 50-368 NOTICE OF VIOLATION Based on the results of an NRC inspection conducted during the period of April 22 - May 21, 1980, it appears that certain of your activities were not conducted in full compliance with NRC regulations and the conditions of your license (NPF-C), as indicated below:

Technical Specification 6.8.1 rquires that, " Written procedures shall be established, implemented, and maintained covering.

The applicable procedures recommended in Appendix "A" of Regulatory Guide 1.33."

Quality Control Procedure 1004.14, Job Orders, has been established in accordance with this Technical Specification.

Step 4.20 in this procedure states ".

. any maintenance work completed requires checkout prior to returning to service.

the cognizant supervisor makes the determination of checkout requirements... the signature of the assigned person of Item 23 (a line item on the Job Order form) indicates that the required checkout is complete and the equipment is ready to be returned to service."

fontrary to the above, the licensee failed to adhere to the pro-cedural requirements relating to the use of Job Orders during the performance of maintenance on High Pressure Safety Injection (HPSI) valve 2CV-5076 (the normally shut valve that automically opens on an Engineered Safety feature signal to allow HPSI flow to 'D' reactor coolant loop).

Specifically, electrical corrective maintenance was performed on valve 2CV-5076 under job order 2-2012-80 which was subsequently documented to be complete and the valve ready to be returned to service on April 7, 1980.

On April 29, 1980, the electrical junction box cover (2TB 903) was found to be missing from valve 2CV-5076 and the junction box door on the supply end was open.

This is an infraction.
