Dear Mr. Keppler:

This letter serves as an interim report as required by 10CFR50.55(e) on the deficiencies concerning Cutler-Hammer motor-control centers. These items were first reported to the NRC by Cutler-Hammer as a 10CFR Part 21 on May 14,1980.

It was later reported as a possible significant deficiency under 10CFR50.55(e) in a telephone conversation between W. J. Kacer and T. A. Boss of The Cleveland Electric Illuminating Company and Mr. J. Konklin of the NRC Region III, Office of Inspection and Enforcement on May 28, 1980 Description of Deficiency Cutler-Hammer was contracted by The Cleveland Electric Illuminating Company to manufacture Class lE motor-control centers for Perry Nuc-lear Power Plant. During final inspection of a portion of these units, it was discovered that the type SIS wire installed in the switch units was not qualified to IEEE 383 and that improper saddle clamps were used on the terminal blocks. Further inspection of all 1,943 starters revealed that 42 contained improper wire and 25 contained terminal blocks with improper saddle clamps. It was later discovered at the

,iob site that four splice plate carriage bolts broke during installa-tion on the bus. Since these deficiencies place serious doubt on the environmental qualification of the equipment, extensive repair will be required to upgrade the units to meet specification requirements.

To meet these requirements, Cutler-Hammer has proposed the following:

1.

Return all starter units with improper wire to the manufac-turer for rewiring.

2.

Replace all termibal blocks with improper saddle clamps.

3.

Replace all splice plate carriage bolts.

CEI feels proposals numbers 1 and 2 are adequate to correct their respective deficiencies. However, we feel that the cause of the carriage bolt breakage has not been adequately addressed. As a result, we will attempt to determine the cause of the breakage in order to determine the adequacy of proposal number 3 At the present time CEI is analyzing the broken carriage bolts in an attempt to i

determine the cause of failure. This evaluation should be completed by July 1, 1980. After that, a plan to resolve the problem will be coordinated with Cutler-Hammer and a final report on these deficiencies will be submitted by September 30, 1980.
