Appendix A NOTICE OF VIOLATION Illinois ~ Power Company Docket No. 50-461 Based on the results of the NRC in=pection conducted on May 20-23, 1980, cit appears that one of your activities was in noncompliance with NRC re-quirements, as noted below. This item is an infraction.

10 CFR 50, Appendix B, Criterion X requires, in part, that a program for inspection of activities affecting quality shall be established and executed by or for the organization performing activities to verify conformance with documented instructions, procedures, and drawings for accomplishing the activity.

Illinois Power Company Quality Assurance Manual, Chapter 10, established the requirements for a program of inspection which provides assurance that fabrication, construction, and installation activities conform to the requirements of the applicable instructions and procedures.

Contrary to the above, the inspector observed inadequate inspection on the containment building concrete work activities for placement CfW-21 on May 20, 1980 in that:

a.

The inspection failed to identify reinforr ng steel that was inade-quately secured in place to prevent displacement by construction loads or placing of concrete, b.

The licensee failed to provide adequate number of quality control inspection personnel in that only one inspector was present for the entire concrete operation including batch plant operations, transporting, placing of concrete, and inspection of reinforcing steel.

c.

The testing laboratory did not provide for temperature monitoring of the field curing facility throughout the storage of test cylin-

ders, d.

The testing laboratory had not identified the concrete test cylinders taken during the placement.

e.

Provisions were not made for the verification that water had not been added'to the truck transporting the concrete material.

The licensee has taken adequate corrective action on each of the above findings as described in Section I of this report. No response is re-quired by the licensee.
