Identification of Deficiency: Containment Vesse' Equipment Hatch Rejectable indications in welding.

Initial Reoort:

On June 12, 1980, Mr C. R.McFarland of NRC Region ll, Atlanta, Georgia, was notified of tnis deficiency by Mr. J. R. Wells of Duke Power Company, Charlotte, North Carolina, 28242.

Supolier and/or Comoonent:

Lakeside Bridge and Steel Company, 5300 N. 33rd Street, Milwaukee, Wisconsin, 53209, is the contract holder for Catawba Unit #1, Equipment Hatch. The Equipment Hatch Barrel is subcontr.scted to Chicago Bridge and I ron, Kankakee, Illinois, CB&l was also re-sponsible for NDE on the Equipment Hatch Barrel .

Description of Defic *ency:

Prior to ir.stallation of Equipment Hatch Barrel, Duke personnel performed NDE which raised questions on vendor welds. Lakeside and CB&l personnel were brought to Catawba site, and all Equipment Hatch Barrel welds were examined by Ultrasonics (UT) by Lake-side and CB&l. Four rejectable weld areas were found. These four rejectable areas totaled approximately 15 inches of a total of 840 inches of weld.

Analysis of Safety lmolications:

Improper NDE and inspection could allow welds not meeting design requirements to remain undetected.

Corrective Actiel:

I. All rejectable weld areas were ground out and weld repaired in accordance with approved procedures. All repaired weld areas were then examined by UT and found acceptable. This action was completed June 11, 1980.

2. All Lakeside orders were reviewed to determine if additional welds were subject to errors in UT inspection. No other rejectable welds were found.

3 A review of Lakeside's NDE Program was conducted to assure proper control of Lake-side's NDE personnel, procedures, and equipment. Additional surveillance of Lake-side will be performed at frequent intervals to assure proper NDE program imple-mentation.
