May 8, 1980 Docket No. 50-423 AEC-MP3-207 United States Nuclear Regulatory Commission Office of Inspection and Enforcement Regfon i 631 Park Avenue King of Prussia, Pennsylvania 19406 At ten tion:
Mr. B. H. Grier, Di rector Gen tlemen:
MILLSTONE UNIT NO. 3 REPORTING OF DEFICIENCIES IN DESIGN AND CONSTRUCTION This letter provides written confirmation of a phone conversation held with your Mr. McGaughy on April 15, 1980.
We have determined that the problem as discussed below is reportable under 10 CFR 50.55(e)(1)(lii).
As reported to Mr. McGaughy on Apell 15, 1980, our engineer constructor for Millstone Unit No. 3, Stone and Webster Engineering Corporation, reported to Northeas t Nuclear Energy Company that examination of the component cooling heat exchanger support steel weldements indicated they were not in con fo rinan ce with purchase specifications.
The component cooling heat exchanger support steel was fabricated by Tower I ron Works.
Visual receipt inspection noted unsatisfactory welds as per the fabrie:ation s peci fi ca t ion.
Tower i ron Works was informed and the material put on " hold."
This receipt inspection nonconformance date was May 29, 1979 The steel supports for the three heat exchangers were installed under risk release No. 104 in August 1979 i

U.S. Nuclear Regulatory Commission AEC-MP3-207, Page 2 May 8, 19 80 Subsequent cessation of business by Tower I ron Works requi red a thorough inspection of the weldements to be completed by Stone and Webster Engineering Corporation. All weld deficiencies were noted and recorded in nonconformance and disposition report No. 0169
.The unacceptable welds found in the supports of the reactor plant component cooling heat exchangers Indicate possible loss of the function of the heat exchangers should the welds fall.
The consequence of the loss of the heat exchangers would be an inability to maintain the plant in cold shutdown.
For these reasons, it is being reported as a significant deficiency.
Disposition details indicate directions for rework.
All rework will be done on site or new items fabricated off site in accordance with the original speci fi ca tions.
Final disposition will have repaired or replaced the sup ports.
Final disposi tion date is not known at this time.
Very truly yours, NORTHEAST NUCLEAR ENERGY CQ PANY
